Position Overview:

As a Care Coordinator at Art of Intelligent Minds, you hold a pivotal role in ensuring our clients receive comprehensive and effective support throughout their treatment journey. This position involves close collaboration with the treatment team, caregivers, medical professionals, and school officials to provide personalized care and foster collaboration aimed at achieving the best possible outcomes for our clients. The Care Coordinator's responsibilities extend to ensuring a clear understanding of the psychologist's diagnosis and treatment recommendations, as well as assisting both the member and the treatment team in securing the necessary resources and treatment as prescribed by Dr. Cunningham

Responsibilities

Direct Client Updates:

Establish and maintain regular contact with clients, meeting at least once a month to obtain updates on their progress and well-being.

Information Gathering:

Collaborate with the individual's treatment team, school officials, parent/caregivers, and other relevant stakeholders to collect essential information and updates to ensure a holistic understanding of the client's situation.

Assessment and Evaluation:

Administer assessments at regular intervals to assess client progression, regression, and the effectiveness of their treatment.

Resource Provision:

Provide necessary resources and support to the client's family throughout the treatment process.

Communication Facilitation:

Ensure clear communication of psychologist findings and recommendations to the client, their family, treatment team, and others involved in the well-being of the member to foster understanding and collaboration.

Resource Connection:

Assist clients and families in connecting with recommended resources and treatment interventions by psychologist Dr. Holly Cunningham.

Collaboration and Coordination:

Facilitate collaboration among the treatment team, caregivers, and school officials to implement psychologist-recommended treatment strategies effectively.

Record Maintenance:

Keep electronic records up-to-date with accurate and recent information concerning the client's progress and treatment.

Supervision:

Receive direct, in-person, individual, and group supervision from Dr. Holly Cunningham, to maintain adherence to professional standards, ensure the highest quality of care, and facilitate your continuous professional development

Assessment and Plan Updates:

Make necessary updates to the client's assessment, diagnosis, and treatment plan as required based on ongoing assessments and client progress.

Qualifications:

Bachelor's degree in a related field or 4 years experience in a related field.

Strong interpersonal and communication skills.

Knowledge of mental health assessment tools and procedures.

Ability to work collaboratively with a diverse team of professionals.

Attention to detail and organizational skills.

Compassion and empathy in working with individuals and families.

Your responsibilities extend to ensuring a clear understanding of the psychologist's diagnosis and treatment recommendations. You will also assist both the member and the treatment team in securing the necessary resources and treatment as prescribed by the psychologist. This role includes direct, in-person, individual, and group supervision from the clinical psychologist ensuring adherence to professional standards and the highest quality of care, as well as facilitating your continuous professional development.
