Requirements and Responsibilities For Case Manager - Home Care
Coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge in a home care environment. Acts as the liaison between patient/family and homecare personnel to ensure necessary care is provided promptly and effectively. Must be a registered nurse with 2-4 years of clinical experience. Familiar with standard concepts, practices, and procedures within a Home Care Agency. Proficient in OASIS D and Quality Assurance. Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks. A certain degree of creativity and latitude is required. Typically reports to a Director of Clinical Services.

Job Type: Full-time

Pay: $35.00 - $45.00 per hour
