REPORTS TO : Resident Services Director, Administrator or Designee

JOB SUMMARY :

Responsible for performing assigned social work duties and responsibilities within the facility. Assist the Resident Services Director to plan, develop, and organize the Resident Services Department in accordance with current Alden policies and procedures, Federal, State and local standards, guidelines and regulations.

Implements program services and individual supports according to prescribed individual program plan and organizational requirements.

Assists residents in personal hygiene and ADL skills : shaving, grooming, feeding, nail care, etc. while using instructional methods according to the individual needs and goals to obtain further independent functioning.

QUALIFICATIONS

Bachelor’s Degree in Human Service field (Social work, Psychology, Sociology, or related field).

Must possess current CPR certification or become certified within ninety (90) days of employment.

Current knowledge of Federal and State Long-Term Care (LTC) regulations and / or ability to understand and interpret.

Demonstrate skills and ability in working with and understanding the needs of residents, families, other staff members and personnel from community agencies.

Must be able to read, write, and speak the English language in an understandable matter. The ability to produce concise, meaningful, assessments, documentation, and articulate the needs of the residents.

Must possess the ability to make independent decisions when circumstances warrant such action that are in the best interest of the residents, staff and families.

Must possess the ability to deal tactfully with residents, family members, visitors, personnel, government agencies / personnel and the general public in a professional manner.

Must possess leadership ability, under the direction of the Resident Services Director, and a willingness to work harmoniously with other personnel.

The ability to accept and utilize professional supervision, consultation, and in-service training / educational opportunities.

Must demonstrate the ability to handle confidential data with professional discretion.

Must be able to relate to and work with the ill, disabled, young, emotionally upset and at times, hostile people.

Must be able to lift, push, pull, and move equipment, supplies, resident in excess of fifty (50) pounds throughout the day.

Is subject to exposure to infectious waste, diseases, conditions, etc., including exposure to COVID, the (HIV) AIDS, and Hepatitis B viruses.

JOB DUTIES

Train / coach / assist residents in communication skills including eye contact, tracking speaker or object, language developmental, use of augmentative communication devices, etc.

Accompany clients on medical appointments, therapeutic community activities, appointments or events away from the facility using necessary vehicles or public transportation in accordance to individual needs, goals, planned events or in emergencies following instructions to maintain safety, health and welfare.
