NFP Registered Nurse Home Visitor Part-Time (32hrs/wk)

This role incorporates health education, physical assessment, health screenings, case management, and linking clients to community resources.
The Region 10 Nurse-Family Partnership (NFP) program has been serving families in Montrose, Delta, Gunnison, Ouray, and San Miguel counties for the last 22 years.
We currently have a part-time position open for a Nurse Home Visitor (NHV) in the Region 10 NFP program. The NHV is responsible for:
Providing comprehensive nursing Services to women and their families eligible for the NFP program.
Maintaining the highest standards in clinical nursing practice and adherence to the NFP model, policies and procedures, guidelines, and standards of Montrose County.
Providing home visits to families while carrying a caseload of 20 clients. We enroll clients during pregnancy and work with them up until baby turns 2 years old.
