**Job Summary:**
We are seeking a dedicated Case Manager to join our team. The Case Manager will be responsible for coordinating patient care and providing support to individuals in need. This role requires strong diagnostic evaluation skills, patient assessment abilities, and a commitment to maintaining patient confidentiality in accordance with HIPAA regulations.
**Duties:**
- Conduct diagnostic evaluations to assess patient needs and develop individualized care plans
- Provide case management services to ensure patients receive appropriate care and support
- Collaborate with healthcare professionals to coordinate treatment plans and services
- Offer addiction counseling and support to patients struggling with substance abuse issues
- Utilize utilization management techniques to optimize patient outcomes and resource utilization
- Maintain accurate and confidential patient records in compliance with HIPAA guidelines
- Deliver exceptional patient service by addressing concerns and providing guidance as needed
**Skills:**
- Proficient in case management practices and patient care coordination
- Strong patient assessment skills to determine individual needs and treatment plans
- Knowledge of addiction counseling techniques and resources for patients in recovery
- Familiarity with utilization management strategies to enhance healthcare delivery efficiency
- Understanding of social work principles to support patients in challenging situations
- Ability to apply physiology knowledge to assess patient conditions accurately
- Excellent communication skills to interact effectively with patients and healthcare providers
- CDCA required**
