Advanced Health Care of Landerhaven is currently seeking a Full-Time RN MDS COORDINATOR for its 46 bed all Sub-Acute – Skilled Nursing Facility.

Candidate must be a Ohio Licensed, RN/LPN in good standing. The RN/LPN MDS Coordinator is responsible for ensuring accuracy of the Minimum Data Set, case management for concurrent reviews, and other nursing duties as needed. Nurse will be responsible for all aspects of patient care, including medication and treatment administration. Nurse will work closely with patient, families, physicians, and staff to deliver excellent patient care. Must be comfortable working with computers, speaking on phone with outside agencies to receive report and arrange care, comfortable communicating with physicians; patients, and family members. We are looking for someone with excellent communication skills, knowledge of PDPM and reimbursement, and wonderful customer service.

Preferably have previous skilled nursing / sub-acute experience.

Schedule:

• 8 hour shift
• Monday to Friday
• Weekends as needed
