Early Intervention Professional-PT

Encompass Community Supports, formerly known as Rappahannock Rapidan Community Services (RRCS) is hiring an Early Intervention Professional to work in our Culpeper office. Encompass Community Supports is a Virginia State community services organization that improves the quality of life in Culpeper, Fauquier, Madison, Orange, and Rappahannock Counties by providing comprehensive mental health, substance use, developmental disability, housing, children’s and aging services. GENERAL DEFINITION OF WORK:

Provides early intervention services to children aged birth to three years of age at-risk for or exhibiting developmental delays; work is performed under regular supervision.

ESSENTIAL FUNCTIONS:

  • Provides service coordination services to infants and toddlers, and their families in home and in community settings, which includes: maintaining contact with families through home visits and phone calls to address ongoing needs and concerns; coordinating and scheduling appointments with other service providers; coordinating and leading meetings with families and other service providers in home and in community settings; assisting families in finding and accessing community resources; conducting intakes in home and in community settings, includes all RRCS documentation and Part C requirements; development of individualized family service plan, including ongoing reviews, and annual updates; completes all Agency and Part C required documentation for discharge, includes face to face meeting in home with family.
  • Designs and delivers direct early intervention services to infants and toddlers and their families, in home & community settings; collaborates, consults, & supports other service providers in providing Part C services.
  • Completes developmental assessments in various settings.
  • Conducts and completes eligibility determination process for infants & toddlers.

other FUNCTIONS:

  • Completes all annual RRCS documentation requirements and entry of information into state Part C database; faxes authorizations, Individualized Family Service Plans (IFSPs), and requests for records to physicians and Medicaid; faxes or mails requests for medical records to hospitals and other medical facilities; makes photocopies and mails copies of paperwork to families and service providers.
  • Receives and processes referrals, which includes, contacting families to provide program information via phone & mail, collect history, and start the Agency’s electronic and Part C charts; entry of information into state database (TRAC-IT).
  • Coordinates community playgroups and other community activities as assigned
  • Participate in community events and coalitions
  • Provides training, information, as well as professional development activities to other Part C providers.
  • Other duties as assigned.

KNOWLEDGE, SKILLS AND ABILITIES:

  • Thorough knowledge of assessment tools, curriculums, training materials, instructional strategies appropriate for infant/toddler intervention.
  • Thorough knowledge of behavior management, “knowledge and consideration of various adult learning styles.
  • Thorough knowledge of service delivery practices and toddlers with developmental disabilities and their families.
  • Thorough knowledge of developmental stages for infants and toddlers.
  • Skill in assessing and collecting client data for program eligibility.
  • Skills in organizing, coordinating and scheduling services with families, other agencies, vendors, etc.
  • Ability to communicate ideas effectively both orally and in writing.
  • Ability to maintain effective working relationships with associates, clients, families, school personnel, service providers and the general public.
  • Knowledge of MS Office, to include Word and Excel.

EDUCATION AND EXPERIENCE:

Current licensure in a field as outlined in Part C Practice Manual to meet criteria for an EI Professional role (ex. active teaching license, SLP/OT/PT licensure); Virginia Part C Certification as an Early Intervention Professional and Service Coordinator; at least 1 year of experience with birth to 3 -year- old population preferred. Bilingual English/Spanish is preferred.

SPECIAL REQUIREMENTS:

Valid Virginia driver’s license; attends training and maintains certifications as required by the Agency and regulatory guidelines.
