Buffalo Crossing Healthcare & Rehabilitation Center is currently looking for a few compassionate, vibrant and dedicated MDS Coordinator to join the team for this brand new facility off of 466 in The Villages, FL.

RESPONSIBILITIES:

• Conduct and coordinate the development and completion of resident assessments, in accordance with the requirements of State, Federal and Company guidelines
• Responsible for overseeing the generation of Minimum Data Set (MDS) for each Medicare patient and electronic transmission of required data within time frame mandated by the State
• Develop, implement, and maintain an ongoing quality assurance program for the resident assessment/care plans
• Develop and participate in the planning, conducting, and scheduling of timely in-service training classes that include assessment skills or techniques needed to complete the assessment and MDS functions
• Oversee the interdisciplinary team in regard to MDS, CAA, and care plan activities for assigned residents
• Attends care plan meetings
• Assign responsibility for completing sections of the MDS to staff who have clinical knowledge about the resident
• Assure that the care planning component is developed with input from staff, physician, resident and families
• Transmit MDS information to the State for all patients in the facility and review lists of residents to determine when the next assessment is due.
• Develops schedule of due dates for periodic assessments.
• QUALIFICATIONS
• Must be a Nurse with required State license in good standing
• Must have at least one year skilled nursing facility experience
• Must be knowledgeable in general nursing standards of care and medical practices.
• Excellent analytical and deductive reasoning skills
• Computer literacy and comprehensive understanding of electronic documentation program.
• Capable of performing basic computer skills including keyboard and data processing.
• Outstanding communication and interpersonal skills
• Excellent technical, assessment, documentation, and writing skills
• Organized and detailed in work performance
• Must be capable of maintaining regular attendance
• Must have patience, tact, enthusiasm, and positive attitude toward the elderly and disabled
• Must be able to speak, read, write, and understand English
• Must be capable of performing the ESSENTIAL JOB FUNCTIONS and PHYSICAL AND SENSORY REQUIREMENTS FOR ALL JOB FUNCTIONS
