Job Responsibilities:

• Assessments and Medication management.
• Proactively manages the ongoing client relationship. Uses independent judgement to accomplish problem resolution, increasing support for clients, and quality assurance. Anticipates needs and advises families.
• Assists with regular and routine assessments for current clients, including identifying and making recommendations.
• Coordinates care with the Client Care Team, Family Members, and other care providers.

Job Requirements:

• Unencumbered and active RN License
• Healthcare Industry experience is required.
