Case Managers work with individual clients and/or legal guardians to assess the client’s strengths and needs while also collaborating with the identified client to connect them with community resources that focus on promoting wellness and independence. The primary components of this position are to coordinate, advocate, monitor, evaluate, refer, support, and assess individualized services being provided, as well as services that are needed on an ongoing basis. Case Managers receive recurring clinical supervision and training revolving around best practices, ethics, harm reduction, diversity awareness, trauma-informed care, person-centered approaches, and documentation enhancement skills.
Duties:
- Provide case management services to individuals and families in need
- Conduct assessments to determine client needs and develop individualized service plans
- Coordinate and monitor client progress toward achieving goals
- Collaborate with other service providers and community resources to ensure comprehensive support for clients
- Advocate for clients' rights and access to necessary services
- Maintain accurate and up-to-date case records and documentation
This position requires strong interpersonal skills, the ability to work independently as well as part of a team, excellent organizational skills, and a commitment to providing quality services. Must have MHRT C or MHRT C/P Certificate for Adult Case Management. Must have a B.A. or B.S. in the related field for Children’s Case Management
Please note that a valid driver's license, reliable transportation, and the ability to pass a background check are required for this position.
Job Types: Full-time, Part-time
Pay: $30.00 - $35.00 per hour
Expected hours: 20 – 40 per week
