The clinician would be responsible for the following:

• Manage a patient caseload
• Coordinate patient care with treatment team and patient’s support systems
• Conduct biopsychosocial assessments and group/individual therapy sessions
• Maintain contact with patient family/support systems
• Monitor patient progress in treatment and develop/update individualized care plans
• Participate in treatment team staffing and document patients status/progress
• Perform case management and discharge planning as directed by the treatment team
• Assist in patient supervision and care on facility grounds.
• Completes documentation thoroughly and timely on a daily basis

Qualifications and Skills

• Master’s Degree in Social Work or Counseling
• Active professional license
• Strong problem solving and group work leadership skills
• Ability to interact with people of all ages and cultural backgrounds
• Ability to work independently and as part of a team
• Sound computer skills
• Effective oral and written communication skills
• Experience working with individuals with chronic mental illness
• Maintain current CPR and CPI training
