Registered Nurse Case Manager
Duties:
- Provide comprehensive nursing care to patients in various settings, including ALF, ILF,
Memory Care, PCH, and residential homes.
- Assess patient needs and develop individualized care plans.
- Coordinate and collaborate with interdisciplinary healthcare teams to ensure continuity of care.
- Monitor patient progress and make necessary adjustments to the care plan.
- Educate patients and their families on disease progression, options of care, resources available for individuals/families facing end of life, symptom and pain management, and wound care
- Advocate for patients'rights and ensure they receive appropriate healthcare services.
- Document patient information accurately and maintain confidentiality.
Qualifications:
- Valid Registered Nurse (RN) license in the state of Texas.
- Minimum 1 year of experience as a Registered Nurse, Hospice experience is not required.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively in a team environment.
- Proficient in electronic medical record (EMR) systems.
- Knowledge of healthcare regulations and best practices.
