Job Summary We are seeking a skilled and compassionate Licensed Practical Nurse (LPN), Licensed Vocational Nurse (LVN), or Registered Nurse (RN) to join our team as an In-Home Care Consultation Nurse. In this role, you will provide personalized consultations to clients seeking home care services. You will assess the individual needs of clients, develop care plans, ensure all necessary documentation is completed, and input relevant information into our computer system. Responsibilities
Client Consultations: Conduct in-depth consultations with clients and their families to assess their home care needs, preferences, and goals.
Assessment and Evaluation: Perform comprehensive assessments of clients' physical, emotional, and social needs. Evaluate clients' current health status, medication management, mobility, safety concerns, and support systems.
Care Planning: Develop personalized care plans tailored to the unique needs and preferences of each client. Collaborate with clients, families, and other healthcare professionals to establish realistic goals and interventions.
Documentation and Record Keeping: Ensure accurate and timely completion of all required documentation, including consent forms, assessment tools, care plans, and other relevant paperwork. Maintain detailed electronic records in the company's computer system.
Education and Support: Provide education and support to clients and their families regarding home care services, resources, and available community support programs.
Communication: Maintain open and effective communication with clients, families, caregivers, and other members of the healthcare team to coordinate care and address any concerns or changes in clients' conditions.
Compliance: Adhere to all regulatory standards, policies, and procedures related to home care services, including HIPAA regulations and state licensing requirements.
Qualifications
Current, unrestricted LPN, LVN, or RN license in the state of Florida.
Minimum of 3 years of clinical experience in a healthcare setting, preferably in home care, geriatrics, or a related field.
Strong assessment, critical thinking, and problem-solving skills.
Excellent communication and interpersonal abilities.
Proficiency in computer skills and experience with electronic health records (EHR) systems.
Ability to work independently and manage time effectively.
Compassionate, patient-centered approach to care.
Reliable transportation and willingness to travel to clients' homes.
