The Lodges operates 12 residential care homes that offer 24 hour assisted living services. Each home accommodates 5 -14 residents comfortably. QUALIFICATIONS:

  • Must be licensed with the State of Minnesota as a LPN and must maintain current and unencumbered licensure.
  • Must live out company’s values and maintain values score of 4 .
  • Must have reliable transportation to and from work.
  • Must be able to pass a criminal and/or abuse background check,and drug screening.
  • Previous experience working with tube feedings, ventilators, tracheostomy preferred.
  • Previous experience working with mental health preferred.
  • Previous experience leading team trainings and training unlicensed staff preferred

SPECIALIZED KNOWLEDGE AND ABILITIES:

  • This position requires the incumbent to:
  • Possess a sensitivity and aptitude for working with persons with disabilities.
  • Possess skills to communicate effectively with those you supervise, residents, family members and RN Supervisor.
  • Possess leadership / supervisory skills.
  • Must be able to prioritize and organize work effectively and efficiently.
  • Able to work irregular hours, occasional weekends and evenings.
  • Ability to lead trainings as delegated by the Director of Nursing.

ESSENTIAL JOB FUNCTIONS AND TASKS:
Job Function: Work within Assisted Living Licensure including completing essential paperwork and complying with state regulations.
Tasks:

  • Works to assure facility is in compliance with federal state and local standards and regulations. Is available for license surveys.
  • Maintains an up-to-date knowledge of current trends of Assisted Living.
  • Works to maintain privacy and confidentiality of resident, resident’s records, and resident living environments.
  • Is willing to carry a cell phone and be available for notifications.
  • Communicates and documents resident data to staff as needed.
  • Assures residents maintain residency requirements, monitors and recommend alternative placement if necessary.
  • Reviews resident service agreements with RN, as needed.
  • Monitors and maintains resident charts and nursing notes.
  • Communicates any changes in resident’s needs or conditions to physicians, as needed.
  • Monitors and follows through with ordering medications and all medication renewals as well as any necessary follow-up with physicians regarding medications.
  • Checks new medications from the pharmacy for medication administration by unlicensed staff.
  • Communicates in staff communication book any information that staff needs to be aware of.

Job Function: Provide support and address needs for programs and services.
Tasks:

  • Maintain resident files and appropriate documents as required.
  • Monitors residents on an ongoing basis to assure their needs can be met.
  • Assures residents continue to live safely in their unit, monitors and recommends alternative care and services, if necessary.
  • Inform personnel of changes in resident’s needs and conditions.
  • Assists ULP in service delivery.
  • Maintains communication with residents, families, and other staff.
  • Responds to resident concerns or complaints.
  • Lead trainings to other team members as delegated by the Director of Nursing.

Job Function: Participate and function effectively as a team member.
Tasks:

  • Develop and maintain a positive working relationship with staff of The Lodges Company, including providing backup to other staff as needed.
  • Articulate information regarding residents to the team.
  • Provide proactive, constructive participation in staff meetings, committees, etc.
  • Assists other departments with special events and family functions.
  • Be compassionate and work with tact and ethical awareness.
  • Be flexible and adaptable to changing situations.
  • Responds appropriately to safety hazards, fire drills and other emergency situations.
  • Reports injuries for self, staff, residents, or visitors immediately to RN Supervisor.
  • Keeps RN Supervisor informed and involved and advised of needs and problems.

Job Function: Mentorship
Tasks:

  • Complete required Leadership Academy training courses and ongoing trainings/meetings including pre and post coursework as assigned.
  • Hosting introductory experience(s) that make mentee(s) feel welcomed and appreciated.
  • Provide guidance, advice, feedback, and support to the assigned mentee(s) on procedures.
  • Provide mentees with all tools to be able to become a better version of themselves.
  • Identify development opportunities for mentee, and regularly provide feedback to Manager.
  • Encourage participation in mentor-mentee bonding initiatives to facilitate mentee’s social orientation and engagement.
  • Supporting mentorship during personal and professional challenges
  • Lodges Culture champion to mentee(s)
  • Other mentor duties as assigned.

Job Function: Lead responsibilities
Tasks:

  • Responsible for leading and assisting with assisting with special projects outside of the normal scope as assigned by the LALD/Operations Manager and the Director of Nursing.
  • Active participant in interdisciplinary meetings that may be held offsite,
  • Supporting the Director of Nursing with clinical oversight
  • Supporting other locations with clinical oversite and guidance

JOB SUMMARY:
Monday - Friday
This position will ensure that the health and safety of all residents are met. The LPN will assist the Director of Nursing with planning, coordinating, and managing resident services and act within the Assisted Living regulations. The LPN is responsible for the overall direction, coordination, and monitoring of assisted living service delivery under the direction of the Director of Nursing. \ This position is responsible for welcoming, orienting, supporting, and engaging mentees. The position is also responsible for leading and assisting with assisting with special projects outside of the normal scope as assigned by the LALD/Operations Manager and the Director of Nursing.

Job Type: Full-time

Pay: $29.00 - $34.00 per hour
