Santa Cruz Supported Living is Hiring a House Lead Near Santa Cruz, CA

Position: House Lead, Enhanced Community Living Support

Service: The Enhanced Community Living Support team supports an adult with a developmental disability to live successfully in our community. The House Lead acts to both support the client as a direct service worker and assist the leadership team in the client's case management.

Purpose of position: The primary focus of House Lead is:

  • Providing direct-service support to the ECLS client in their home and in the community by providing supervision, training, companionship, behavioral support, and other duties deemed necessary by the client and their circle of support.
  • Provide ongoing direction, communication, and coordination of ECLS services to the client's team. Assure consumer goals and services are documented; and provide initial and ongoing training for ECLS workers.
  • Works as part of the Case Management Team along with House Facilitator, Case Managers, and Director to ensure the integrity and functioning of house systems.
  • In partnership with the Case-Management Team, ensure the client receives all necessary support in a manner consistent with Person-Centered planning and Supported Living regulations.
  • Provide crisis and behavioral support for the client. This may entail helping to cover open shifts.
  • Ensuring client and staff safety by following the client's behavior plan.

Qualifications / Education: The successful House Lead is reliable, energetic, flexible, capable of working both independently and as part of a team and is a positive role model for Supported Living’s clientele and direct-service staff. Qualifications include:

  • Satisfactory oral and written communication, math, computer and critical thinking skills.
  • A high school diploma or its equivalent.
  • Knowledge and experience using technology, including computers, mobile devices, email, MS Word/ Excel, and Google Docs.
  • A minimum of one - two years experience working with adults who have developmental or intellectual disabilities (or similar experience).
  • First Aid and CPR certification and the ability to maintain that certification.
  • Some familiarity with SLS programs and consumers is preferred.

Requirements:

  • Department of Justice, or criminal background check.
  • The position may require a California driver’s license, a good driving record, and a registered and insured vehicle, depending on the needs of the client.
  • Personal Assistance: Ability to assist the individual with activities of daily living including bathing, toileting, and other self-help skills.
  • Challenging Behaviors: Ability and willingness to support individuals with challenging behavior.
  • Ability to read and interpret documents, write specific reports and correspondence, apply mathematical concepts to practical situations, solve complex problems, and to interpret written and verbal instructions is required.
  • Ability to respond professionally and appropriately to crisis situations.
  • Ability and willingness to train and coach direct-service staff in order to improve the provision of services to consumers of services from all team members

Reports To: Case Manager

Status: Non-exempt, hourly, full-time position. Occasional overtime as needed.

Work Schedule: Flexible hours as required by the consumer, staff, and program. Staff must be available during business, evening, weekend, and holiday hours.

Compensation:

$32.00/hr to $ 35.00/hr

Duties / Responsibilities:

  • *Ensures consumer safety in the home and community while on duty with the consumer and by monitoring direct service staff.
  • *Assists consumer in identifying and achieving personal, behavioral, educational, or vocational goals as per the Individual Support and ISP Programs and Behavioral Programs.
  • *Assists with household duties such as cleaning, cooking, and household management to maintain the household in accordance with the guidelines determined by the consumer, and consumer’s circle of support.
  • Provides necessary support for consumer to meet personal needs, such as grooming & hygiene.
  • *Provides skills training for assigned consumers and ongoing training to direct-service staff, when applicable.
  • *Participates in all mandatory training.
  • *Responds to crisis in accordance with Santa Cruz Supported Living’s policy and training.
  • *Assists consumer and Facilitator/ Case Manager with identifying and responding to any medical needs, scheduling appointments, planning/ preparation for appointments (such as gathering any documents that need to be brought to the appointment), as requested.
  • *Provides support to consumer in the home and community.
  • *Provides training and coordination of direct-service staff, working with Case Management Team to highlight needed improvements and to identify and implement solutions
  • Monitors all in-home systems (for instance, may include: monitoring usage of online documentation of services provided by assigned Direct-Service staff, medication-administration, budgeting/ cash handling/ tracking, menu planning, housekeeping plans, calendars) for utilization and compliance by SLS team; communicates proactively with Facilitator and Case Manager about any issues identified & works collaboratively to develop and implement a plan.
  • *Responsible for the review and upkeep of all household safety/disaster prep supplies.
  • *Communicates and coordinates services with direct-service staff, BCBA Team, Case Manager, and Associate Director, acting as a hub for channeling important communications
  • Develops and maintains written documentation on a weekly, monthly, quarterly and annual basis, as required, including weekly SLS updates, weekly consumer financial review, Monthly Safety Inspection, partnering with Case Management Team to develop quarterly and annual reports, and other documents and communications as is deemed necessary.
  • *Maintains House Calendar: records all planned activities and meetings on house schedule, works with Case Manager to ensure all scheduled medical appointments are included on all relevant calendars; and posts any other Santa Cruz Supported Living correspondence to the Direct-Service Team.
  • *Updates house binders at least once a month, ensuring all paperwork (office forms, data collection forms, medication information, etc) is up to date and available.
  • *Complies with Santa Cruz Supported Living’s goals, policies, and procedures.
  • *Makes all decisions based on agency Vision and Values Statement.
  • Will be available by phone during business hours to help support on-duty direct service staff.
  • Will assist with scheduling, including finding coverage for open shifts and covering open shifts as needed.
  • Completes all other duties as assigned by supervisor.
