Are you ready for a leadership role in a person-centered agency supporting adults and children experiencing intellectual and developmental disabilities throughout NW Oregon? We are adding a Support Coordinator to our administrative team. We are looking for someone with excellent people skills who is highly organized and computer savvy to work directly with individuals to coordinate their support needs with available staff.

As the Support Coordinator, you will learn the needs of each individual through conversation with the person and reading their support documents. You will write a safety plan for the person to ensure staff will know how to address identified risks. You'll also conduct meet and greets to match people with staff based on needs, preferences, and personality fit. When a match is made, you will enter detailed data for both the individual and staff into Therap, our electronic health record, and set up and maintain a work schedule for the staff. You will review the support documents with staff and train them to work independently. On a monthly basis, you'll write a progress report, sharing with the case manager how the staff is working with the individual on their desired outcomes.

The Support Coordinator will work out of our Clatskanie, Oregon, office. Travel for meet and greets is a requirement of this job. All needed training will be provided.

Qualifications:

• Education/experience in the I/DD field preferred
• Must be able to pass a background check
• Be able to read and write clearly and use multiple computer platforms
• Current valid driver’s license and good driving record
