Overview

Taking part in community health is rewarding and meaningful. LifeLong Medical Care is looking for a Physician or Advanced Practitioner committed to providing high-quality care to patients seen in our Homeless Services Department. The ideal candidate must be dedicated to building a collaborative team model of care and have experience working with the under-served.

LifeLong is always interested in recruiting top talent to fill either part-time or full-time positions across multiple sites in its Homeless Services Department. Please submit an application if you are interested in working in any of our programs.

LifeLong's Homeless Services Department encompasses several programs at the cutting edge of innovative health services. You could work in anyone of these sites, depending on openings:

    Street Medicine: LifeLong is the largest contracted provider of Street Medicine services with six teams that span across all of Northern Alameda County. Services are entirely field-based, and you can expect to see patients in their own environments such as under bridges or overpasses, in encampments, and on the streets. Street Medicine positions are usually part-time.
  • Medical Respite: Medical respite is a unique program that provides housing, and semi-acute medical and nursing care in a 27-bed facility. Respite takes patients experiencing homelessness who are too ill or frail to recover on the streets but are not ill enough to be in a hospital. Providers are critical members of the care team by collaborating with our nurses to ensure high quality medical care, communication, and patient satisfaction. Medical Respite positions are usually part-time.
  • Primary Care: We offer high-quality outpatient primary care at our Trust Health Center in Downtown Oakland. Trust utilizes a multidisciplinary team approach with primary care providers, RNs, Social Workers, Psychiatry, Podiatry, etc. Providers working at Trust can expect to provide a range of scheduled and drop-in preventative, urgent, and primary health care services. Trust positions are either full-time or part-time.
  • Supportive Housing: Our Supportive Housing Program (SHP) offers comprehensive wraparound case management and housing retention support to individuals living in permanent supportive housing after experiencing a period of homelessness. We have several satellite clinics that provide primary and urgent outpatient care to residents living in buildings served by LifeLong. SHP positions are usually part-time.
  • Drop-in Services: Our Place is a drop-in center for people experiencing homelessness, at-risk of homelessness, or who are recently housed that provides an array of outpatient and drop-in medical and psychosocial services in West Oakland. There is a small part-time clinic that provides preventative, urgent, and primary care services. Our Place positions are usually part-time.

Case management and behavioral health services are fully integrated into our model of care at all sites to address the full range of medical and psychosocial factors impacting the overall health of the patient.

Benefits:

Compensation: $197,000-$218,000/year for MD/DO and $123,000-$146,000 /year for APP and pro-rated depending on years since licensed, sign-on bonus, and excellent benefits: medical, dental, vision, Flexible Spending Accounts, dependent and domestic partner coverage, 403(b) retirement savings plan, and loan repayment programs. LifeLong is a qualified site for federal and state loan repayment programs (HPSA score is 21). Acceptance rates may vary year to year based on funding levels and the number of applicants.

Mandatory COVID-19 Vaccine Policy:

Responsibilities

  • Provides preventive, urgent care and primary care health services.
  • Performs clinical assessment of patients, including ordering appropriate diagnostic evaluations, and plans a program of care.
  • Provides verbal and written education and counseling to patients, families, and caregivers.
  • Plans follow up care including requests for outside consultations and evaluations.
  • Maintains patient records necessary for clinic reporting, billing and quality improvement.
  • Functions potentially in a variety of settings including health center, skilled nursing facilities, supportive housing sites, and in clients' homes.
  • Participates as a member of the site clinical services team and attends clinical team meetings.
  • Provides specialty care services, clinical case management and team participation to designated programs (e.g., HIV services, women's health, house calls, diabetes, Bridge, etc.)
  • Coordinates care with other health team members including case management and behavioral health staff.
  • Acts as an advocate for the patient and liaison with outside agencies as appropriate.
  • Physicians supervise Advanced Practice Providers
  • Participates in after-hours on-call as needed.
  • Assists in the supervision and teaching of medical residents, students or clinical fellows.
  • Actively participates in internal quality improvement teams and works with members proactively to drive quality improvement initiatives in accordance with the mission and strategic goals of the organization, federal and state laws and regulations, and accreditation standards.

Qualifications

  • Commitment to the provision of primary care services for the underserved with demonstrated ability and sensitivity in working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic and cultural origins.
  • Ability to take initiative, work independently and make sound judgments within established guidelines; understand and apply oral and written instructions; establish and maintain effective working relations with staff, clinical providers, managers and external agencies or organizations.
  • Excellent interpersonal, verbal, and written skills and ability to effectively work with people from diverse backgrounds and be culturally sensitive.
  • Be creative and mature with a "can do," proactive attitude.
  • Strong organizational, administrative, multi-tasking, prioritization and problem-solving skills.
  • Ability to work effectively under pressure in a positive, friendly manner and to be flexible and adaptive to change.
  • Work in a team-oriented environment with a number of professionals with different work styles and support needs.
  • Conduct oneself in internal and external settings in a way that reflects positively on LifeLong Medical Care as an organization of professional, confident and sensitive staff.
  • Ability to continuously scan the environment, identifying opportunities for improvement and intersections with other departments of LifeLong Medical Care and partner organizations.

Job Requirements

  • Current California MD, PA, or NP license.
  • Training and experience in geriatrics, internal medicine, and/or family practice with board eligibility or certification required.
  • Interest in working with a diverse patient population in a community based organization and sensitivity to their needs and health problems; commitment to a community health philosophy of prevention and primary care.
  • Experience with electronic health records.
