Georgia Department of Public Health Commissioner and leaders encourages all employees to engage in regular wellness activities and to make lifestyle choices that promote health and well-being. The use of wellness breaks during the workday is authorized to support this philosophy and assist employees in meeting their wellness goals. A maximum of 30-minutes in a given workday may be used to engage in wellness activities, generally in the form of two 15-minute breaks or one 30-minute break.

You will support the Department by utilizing your expertise to fulfill the following responsibilities:

Identify high risk populations and tailor interventions based on the needs of the population and resources available.
Provide home visiting services to identify, assess and provide care coordination services to eligible infants and children at risk for poor health and developmental outcomes.
Coordinates program services with other service providers and community organizations to promote optimal implementation.
Screen and refer clients to other community service providers.
Provide referral/linkage for children and families with identified medical and environmental risks/needs to appropriate services.
Monitoring per program guidelines, educational instruction and materials to promote wellness, safety, child development and disease prevention.
Conduct comprehensive assessments of client's health care needs.
Develop plans of care for individuals, families and populations. Modify treatment plans as indicated by client's responses and conditions.
Provide health education instruction and/or classes to individuals, and families, for the promotion and maintenance of health.
Promote all Child Health programs through community engagement and outreach events. Develop and sustain community partnerships.
Participate in drills and exercises on pandemic influenza, mass sheltering and mass dispensing.
Other duties as assigned.
Maintain knowledge of and seek out new community resources.

The East Central Health District services 13 counties

Qualifications:

Bachelor's degree in nursing from an accredited college or university AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification, OR associate degree in nursing from an accredited. college or university AND One year of nursing experience AND current
Georgia Registered Professional Nurse license AND CPR certification.

Preferred Qualification:

Must have the ability to work independently, be self-motivated and have exceptional time-management, interpersonal and organizational skills.
Have reliable transportation. Travel is reimbursed at the State of Georgia mileage rate.
Public Health and home visiting experience
Pediatric experience, preferably those with special health care needs
Familiar with M&M and SendSS software

Job Type: Full-time

Pay: $45,900.00 - $61,350.00 per year
