I. Qualifications
Candidate must be a graduate of a state certified Nurses Aide training program and in good standing with the State Board of Nursing. Candidate must attend first available Volunteer Training Program. Experience with death or dying patients is helpful. Candidate must embrace the hospice philosophy. Current drivers license, auto insurance and a dependable means of transportation required.
II. Responsibilities
The CNA works under the direct supervision of the RN Case Manager and/or Patient Care Coordinator to:
-Assist with personal care of patient, i.e., bathing, feeding, bed linen care, mouth care, vital signs, transferring and shaving.
-Document in patients chart.
-Follow infection control and safety measures.
-Assist with patient/family teaching concerning patient care and care for the caregiver.
-Bereavement activities as requested and/or able.
-Clean and monitor supplies.
-Attend and participate in staff and patient review meetings when able.
-Maintain and uphold the Hospice Code of Ethics.
-Work as a Hospice of the Premier care Hospice Team member.
-Keep Patient Care Coordinator/Nurse Case Manager informed of condition of patient/family.
-Maintain appropriate number of supplies at patients residence.
-Provide emotional support to patient/families.
III. Orientation and Training
Lily Care of NM will provide orientation and training, as necessary, to assist the CNA in performing the responsibilities listed above. Trainings may be in-house, or outside seminars or approved self-study. Documentation of orientation and training will become part of the personnel file.
IV. Accountability
Reports directly to the Nurse Case Manager, and/or Director of Nursing
