Nurse Practitioner

We are seeking a qualified NP candidate to join our team who will work in our Rose and outpatient dialysis clinics. NP will care for Colorado Kidney care office patients as well outpatient dialysis patients. All candidates must be able and willing to travel to different work locations regularly; reliable transportation is required.

We offer a competitive pay range; annual salary offered at $99,000 to $101,000 for full time exempt position, commensurate with qualifications and experience. Excellent benefits package offered.

Employment at Colorado Kidney Care is contingent on successful completion of pre-employment screening and background check. Vaccination for COVID-19 is required. Employees will provide proof of Vaccination upon hire.

Position Responsibilities and Duties:

• Perform inpatient consults and follow-up visits to our patients within long term care and hospital facilities.
• Chart all patient visits in the LTAC facility’s EHR system. This includes orders entry and any other actions that need to be taken for patient care plan.
• Provide patient education to communicate physician instructions regarding; medication changes, requested diagnostic follow-up/labs, diet changes, next visit, etc.
• Complete billing entry for services in the ECW mobile application the same day services are rendered.
• Assist with patient coordination of care regarding hospital discharge, dialysis, transplant, access and dietary services.
• Communicate with daily covering LTAC & hospital physicians regarding patient care issues and concerns. Additionally, check-in daily with covering MD to confirm which patients to assist rounding on, and other tasks that can be assigned to you.
• Entry, Review, Refill and Maintenance of medications in hospital EHR
• EHR software patient care data entry must be accurate and timely; you are expected to document all patient care interactions and pertinent medical record information in the EHR charts.
• Maintenance and upkeep of incoming messages via EHR and email; this includes timely responding and follow up

• Answering and triage of phone calls from patients and dialysis unit staff seeking advice/clarification.
• Cross coverage of other duties as needed, and other duties as assigned
• To round, assess and evaluate as directed on each ESRD patient in the dialysis units to which you have been assigned. ESRD patient duties include: chair-side visits and charting of each visit, filling of prescriptions both chair-side and by phone, contacting referring physicians regarding clinical issues, assisting with arrangement of tests when assistance is needed from the dialysis staff to clarify medical issues, and completing annual History and Physicals on ESRD patients as needed.
