Full Job Description

Panorama Healthcare is seeking a Nurse Practioner to treat residents living in long-term care facilities. Candidates must have experience providing care as a nurse practitioner in the long-term care environment. This candidate will be prepared to provide medical services as an independent contractor, can expect to be highly compensated, and should be prepared to travel between one to two nursing homes. The provider will generally round 4-5 days a week and on a schedule that accommodates both the provider and the facility. The work is located in long-term care facilities.

Job Summary:

Providing timely and targeted clinical intervention, treatment, health care provider education and consequential disease prevention, facility support and validation in meeting state and federal health care requirements.

Coordinates the patient plan of care with the physician and the facility’s clinical disciplines to address patient needs, evaluate the effectiveness of care, and assess changes to the patient's medical condition to ensure healing outcome goals.

Recommendations for prescription facilitate prevention and healing.

Educate, support and collaborate with facility staff to reduce the riisk of re-hospitalization.
