Description: Description: Works under the supervision of the Home Administrator. Responsible for the implementation, coordination and supervision of the facility in the provision of in home support and care needs of the clients. ONLY 4 CLIENTS.
Responsibilities:
- Responsible of care assessment, planning and management with focus on in home/community needs of the clients including initial and ongoing assessment of home home environment in relation to changing needs of the clients
- Coordinate information from clinic, day health center and home care staff and family members in the provision of the IHCP or Individual Service Plan
- Actively participate in ensuring the facility meets the needs of the clients including admission, screening, transfer assistance and discharge planning
- Acts as advocate in securing that clients are receiving medical care, including working with IDT, consultant and primary care physician in ensuring that the clients medical needs are met or communicated in a timely manner
- Provide on going assessment in clients home in coordination with the Inter Disciplinary Team, clients primary care provider in developing and implementing follow up care and services as indicated in the clients treatment plan
- Function as a member of the facility management team to promote leadership and program operations
- Responsible for the development and implementation of competency base initial orientation and on going education of licensed staff and direct care providers
- Meets all the administrative requirements including statistical records, medical records, utilizing computerized documentation system, attending staff meetings
- All other reasonably related tasks as assigned

- Has leadership skills to lead a team of direct care providers.
- Ability to effectively communicate with patients, families, and healthcare professionals.
- Excellent organizational skills and attention to detail.
- Ability to work within the health care team with initiative, imagination, resourcefulness and flexibility
- Computer literacy
- Ability to prioritize tasks and manage time effectively.
- Proficient in using medical equipment and technology.
- Required to have DSP1 and DSP 2 Training
- Current Driver's License
