Job Summary:
American Star Home Health & Hospice Care is seeking a detail-oriented Intake Coordinator to join our team. The ideal candidate will be responsible for coordinating hospice patient intake and scheduling appointments in a medical setting for our Lubbock and surrounding area hospice patients.
Duties:
- Collect and verify patient demographic and insurance information for hospice care
- Schedule appointments and follow-up visits efficiently
- Coordinate referrals to specialists as needed
- Maintain confidentiality of patient information in compliance with HIPAA regulations
- Assist with medical records management and documentation
- Provide administrative support to the healthcare team
- Coordinate with the nurses and medical director for quality patient care
- Coordinate with the facilities and for their contractual obligations
- Keep ASHHC's contracts with facilities active
- Coordinate with the DME companies
- Responsible to conduct the IDT meetings
Qualifications:
- Proven experience in medical administrative support
- Knowledge of medical terminology and procedures
- Understanding of HIPAA regulations and patient privacy laws
- Hospice Intake Coordinator experience preferred
- Ability to multitask in a fast-paced environment
- Excellent communication and interpersonal skills
