Position Summary
The Physical Therapist restores patient’s function, alleviates pain, andimproves safety, allowing the patient to continue to remain living in thecomfort of home. In the home health care setting, the Physical Therapistfunctions as a piece of the interdisciplinary team, assisting in the patient’scoordination of care.

In this role you will:

• Develop and implement an individualizedtreatment plan based on the patient’s needs and achievable functional goals.
• Instruct patients and family/caregivers aboutexercise programs and safety with mobility within the home.
• Communicate with patients, staff,family/responsible party, physicians, and related individuals regarding anychanges, problems, concerns, and/or special needs the patients may have.
• Travel to, and treat the patient, within theirhome setting.

Qualified candidates willhave:

• Graduate from an Accredited School of PhysicalTherapy approved by the State of Nebraska.
• Possess a valid PT license from the appropriatestate. This shall include maintaining required CEU’s.
• Experience in Home Health Care preferred but notrequired.
• Must be willing and able to become CPRCertified.
• Opportunities to become OASIS certified.
