A Hospice Skilled Nurse is responsible for checking on patients in their houses or assigned facilities on a weekly basis. This only involves mostly assessment of the patient's condition and needs, and coordinating these needs to the main office staff for prompt action, if needed.
A patient home visit would usually be only at a one-hour time frame, and the time of visit may be coordinated between the nurse and the patient.
The Skilled Nurse must have a reliable mode of transportation as the job requires travelling to patient's houses.
The Skilled Nurse may be involved in as few or as many patients possible, depending on the time that the he/she is available.
Occasionally, the Skilled Nurse is asked to report to the Hospice's main office for proper coordination of care, and better communication with the staff.
