Family Support Services

Job Summary

The FSS Social Worker provides crisis intervention and ongoing supportive case management services to individuals and families with the desired outcome of each client reaching their maximum level of self-sufficiency.

Principal Accountabilities

  • Client Social Work - Conduct a comprehensive assessment of needs for each client family and arrange, coordinate, monitor, evaluate, and advocate for an array of multiple resources and services to assist each client family in reaching their maximum level of self-sufficiency.
  • Assessments – All assessments are completed and entered within 5 business days of the intake and/or evaluation dates.
  • Treatment Planning - Develop, in collaboration with each client, an integrated, individualized, and person-centered treatment plan by engaging clients in the establishment of goals and objectives, providing guidance, and monitoring progress throughout treatment. All treatment plans are entered into Evolv within 5 days of the appointment.
  • Compliance - Maintain client documentation and recording by preparing and completing all required assessments, treatment plans, progress notes (within 2 days of client contact), records, Discharge Summaries (30 days) and reports for all assigned clients in accordance with agency standards. Conduct and document 60% productivity standard of direct client service contact time.
  • Ethics - Maintain the standard of social work practice by following the principles of professional ethics and all applicable codes of ethics.
  • Training - Increase professional and technical knowledge by attending agency in-service programs and educational workshops, reviewing professional publications, and establishing personal networks.
  • Ambassadorship - Promote the agency by ensuring an understanding of program services available for clients, publicize activities and accomplishments, and outreach with community members as appropriate.
  • Security and Privacy – Adhere to agency and client protection policies by regularly acclimating yourself with the JFS privacy, safety, and security policies made available within the employee handbook.
  • Employee Philanthropy – Participate annually in agency and partner related philanthropic
  • activities and campaigns.
  • Mission Dedication – Embody the mission and values of JFS in all work done on behalf of the agency. Utilization of the JFS key strategies will ensure success in this accountability.
  • Misc. – Other duties as assigned by management.

Minimum Qualifications

Experience: Minimum of one year case management experience working with individuals and families in a human service capacity. Fluency both oral and written in Russian, a plus.

Education: Minimum of an LBSW/LLMSW, or related degree. LMSW preferred.

Skills

  • Technology - Create, manipulate, and maintain documents in Microsoft Outlook, Word, and Excel, as well as agency electronic medical records and other electronic reporting software which will be used for program maintenance and evaluation.
  • Communication - Express oneself in an articulate and effective manner verbally and in written form.
  • Problem Solving and Reasoning - Deal with abstract as well as concrete and complex problematic situations; define problems, collect data, establish facts and draw valid conclusions.
  • Organizational Skills - Demonstrate a systematic approach in carrying out job responsibilities.

Knowledge

  • Case Management Practice - Apply knowledge of Social Work principles and practice in the therapeutic process, including Motivational Interviewing and Person-Centered Planning.
  • Community Resources - Understand and use community resources on the local, state and federal level in order to enhance client service provision and to maximize agency financial assistance funding.
  • Cultural Sensitivity - Be understanding and sensitive of minority group needs and preferences.
