Job Description: We are seeking a dedicated and qualified individual to join our team as a Developmental Disability Professional (DDP). As outlined in the Provider Manual for DBHDD Community Developmental Disabilities Providers, the DDP plays a crucial role in overseeing the delivery of waiver and state services to participants. The successful candidate will ensure that services provided are in line with individualized service plans (ISP) and maintain a person-centered focus in service delivery.

Responsibilities:

• Overseeing the implementation and delivery of Individual Service Plans (ISP).
• Supervising the delivery of services to ensure alignment with ISP goals and objectives.
• Monitoring progress towards ISP goals and making recommendations for modifications when necessary.
• Conducting functional assessments, including Health Risk Screening Tool, Supports Intensity Scale, and Functional Behavioral Analysis.
• Supervising high-intensity services addressing health and safety risks, such as Behavior Support Plans and Crisis Plans.

Requirements and Designations: Candidates must meet one of the following qualifications:

• Advanced Practice Nurse.
• Behavior Specialist.
• Board Certified Behavior Analysis (BCBA).
• Educator.
• Human Services Professional.
• Master’s or Doctoral Degree Holder.
• Physical or Occupational Therapist.
• Physician or Physician’s Assistant.
• Psychologist.
• Registered Nurse (Associate Degree or Bachelor’s Degree).
• Speech Pathologist or Audiologist.
• Therapeutic Recreation Specialist.

Additional Requirements:

• Completion of required trainings, including Individual Service Planning, Supports Intensity Scale overview, and Health Risk Screening Tool online training.
• Documentation of a minimum of 8 hours per year of additional DBHDD-sponsored or other training in the area of developmental disabilities.
• Must be employed by or under professional contract with the provider agency.

Job Type: Full-time

Pay: $35,000.00 - $45,000.00 per year
