This position functions as a member of the professional staff, providing coordinated occupational therapy to patients of all age groups under the supervision of a licensed Occupational Therapist. The intent of this job description is to provide a summary of the major duties and responsibilities of this position and shall not be considered as a detailed description of all the work requirements that may be inherent in the position JOB DUTIES AND RESPONSIBILITIES: ESSENTIAL FUNCTIONS: 1. Administers the occupational therapy treatment plan as developed by the OTR. 2. Facilitates the patient’s ROM, strength coordination, use of adaptive equipment and environmental modifications, functional mobility and safety issues as related to patients ADL performance needs under the supervision of the OTR. 3. Communicates with the Therapy Supervisor or Clinical Coordinator and appropriate staff in order to plan total patient and family care. Confers with P.T., nursing staff, S.T., MSW and HHA’s in planning rehabilitation for the patient. 4. Observes, records and reports patient’s response to treatment and any changes in patient’s condition to physician and/or clinical supervisor. 5. Documentation is timely, accurate, legible and in compliance with the agency policy and directives. 6. Documents the effectiveness and outcomes of care on an ongoing basis and makes revisions as needed. 7. Plans for discharge as appropriate in conjunction with OTR. 8. Instructs other health team personnel, when appropriate in certain areas of occupational therapy including activities of daily living, in which they may work with the patients. This includes the patient and family members. 9. Applies concepts of infection control and universal precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease. 10. Accepts clinical assignments that are consistent with education and competence to care for patients. 11. Meets mandatory continuing education requirements. 12. Provides orientation to new orientees related to scope of practice. Entity 33 Job Class Code Number 478 13. Maintains confidentiality of all materials handled within the Network/Entity as well as the proper release of information 14. Complies with Network and departmental policies regarding issues of employee, patient and environmental safety and follows appropriate reporting requirements. 15. Demonstrates/models the Network’s Service Excellence Standards of Performance in interactions with all customers (internal and external). 16. Demonstrates Performance Improvement in the following areas as appropriate: Clinical Care/Outcomes, Customer/Service Improvement, Operational System/Process, and Safety. 17. Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, processes and practices. 18. Complies with Network and departmental policies regarding attendance and dress code. 19. Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated OTHER FUNCTIONS: 1. Attends meetings as designated or requested. 2. Participates in case conferences when appropriate. 3. Other related duties as assigned. ADDITIONAL REQUIREMENTS: 1. Ability to provide consultation. 2. Ability to instruct nonprofessionals in the care and treatment of the physically and emotionally ill. 3. Ability to prepare neat, timely, accurate and concise records and reports. 4. Ability to accept and utilize supervision. 5. Possession of a current license to practice as an occupational therapy assistant in the Commonwealth of Pennsylvania. 6. Possession of a phone in residence. 7. Position requires the use of your own private vehicle for Network business. You must have a valid driver’s license issued by the applicable state of residency, as well as evidence of auto liability insurance; which must have at least the state-minimum coverage limits. Insurance must be maintained at all times while being employed in this position. PHYSICAL AND SENSORY REQUIREMENTS: Reference from U.S. Department of Labor Definitions 1991 Heavy Work: Involves exerting 50-100 pounds of force occasionally, or 25 to 50 pounds of force frequently, or 10-to 20 pounds of force constantly to move objects. Constantly standing and walking. Physical demand requirements are in excess of those for Medium, Light and Sedentary work as defined by U.S. DOL. Reaching: Frequent Reaching Above the shoulder: Frequent Twisting: Frequent Bending/stooping: Frequent Repetitive movements of elbows/fingers: Frequent Repetitive movements of wrists: Frequent Kneeling/Climbing /Squatting: Occasionally Good physical and mental health Neat, clean, free from body odors. Conforms to uniform code Visual and auditory acuity are required to provide comprehensive care Frequency Definition Max # hrs in a 8 hour day Occasionally Activity/condition exists up to 1/3 of the time 2 hours 40 min. Frequent Actively/condition exists from 1/3 to 2/3 of the time 5 hours 20 min Constantly Activity/condition exists 2/3 or more of the time. 8 hours Environmental Inside: Office and patient homes Outside: Drive to patient homes/clinics and or office errands in extreme cold/heat Wet and or humid conditions, snow, ice and fog Hazards: Exposure to contagious disease, road and driving and animals. SPECIFIC PROTECTIVE EQUIPMENT AVAILABLE: Gloves, gown, mask, goggles. SUPERVISION (RECEIVED BY AND/OR GIVEN TO): Receives supervision from therapy clinical supervisor and/or OTR’s. • Supervises home health aides. COMMUNICATIONS: 1. Ability to communicate effectively, both orally and in writing, to patients, families, physicians, and related persons. 2. Ability to exercise tact and good judgment in dealing with people. 3. Communication honors and respects ethnic and cultural diversity. QUALIFICATIONS (MINIMUM) EDUCATION: A Certified Occupational Therapy Assistant is a graduate of an accredited school of occupational therapy with an Associate Degree in Occupational Therapy.
