Volunteers of America - Baton Rouge is Hiring a Turning Point Case Manager Near Alexandria, LA
Volunteers of America South Central Louisiana is looking to hire a Case Manager in our Turning Point Re-Entry program in the Alexandria area. The Case Manager is to provide intensive, time-limited services to ex-offenders who will be or have been released from correctional facilities who need housing, transportation, and other basic needs assistance with the goal of developing and executing a comprehensive collaborative case plan to provide or refer services needed for successful re-entry. Essential Functions: Program Services: Responsible for collaborating with a variety of agencies and service providers in the community on both a professional and paraprofessional level for the purpose of providing time-limited supportive services to individuals selected to participate in this program. Job Duties include but are not limited to:
Securing and maintaining potential permanent housing sites.
Contact consumers within three working days of the referral.
Complete assessments and client intake forms within prisons, jails, and other on-site locations.
Provide or ensure provision of support services necessary to enable the participant to maintain community living, such as (but not limited to):
Supportive counseling
Transportation and travel for training
Learning basic daily living skills
Building a personal social network
Support services needed for the consumer to attain vocational goals
Assist the consumer in obtaining benefits/entitlements
Assist the consumer with obtaining legal, behavioral and physical health, education and other services needed
Linkage with other community services
Crisis intervention as needed
Keep accurate progress notes and monthly reviews in accordance with the client's treatment plan.
Assist consumers with moving into their house or apartment. This may include having utilities turned on, assisting with rent, and working with landlords
Any other duties as assigned.
Required Education and Experience: Bachelor's Degree preferred in social service, criminal justice, or a related discipline. Proficient in computer skills, ability to work as a team, and have good interpersonal skills.
