Volunteers of America South Central Louisiana is looking to hire a Case Manager in our Turning Point Re-Entry program in the Alexandria area.
The Case Manager is to provide intensive, time-limited services to ex-offenders who will be or have been released from correctional facilities who need housing, transportation, and other basic needs assistance with the goal of developing and executing a comprehensive collaborative case plan to provide or refer services needed for successful re-entry.
Essential Functions:
Program Services: Responsible for collaborating with a variety of agencies and service providers in the community on both a professional and paraprofessional level for the purpose of providing time-limited supportive services to individuals selected to participate in this program.
Job Duties include but are not limited to:

• Securing and maintaining potential permanent housing sites.
• Contact consumers within three working days of the referral.
• Complete assessments and client intake forms within prisons, jails, and other on-site locations.
• Provide or ensure provision of support services necessary to enable the participant to maintain community living, such as (but not limited to):
• Supportive counseling
• Transportation and travel for training
• Learning basic daily living skills
• Building a personal social network
• Support services needed for the consumer to attain vocational goals
• Assist the consumer in obtaining benefits/entitlements
• Assist the consumer with obtaining legal, behavioral and physical health, education and other services needed
• Linkage with other community services
• Crisis intervention as needed
• Keep accurate progress notes and monthly reviews in accordance with the client's treatment plan.
• Assist consumers with moving into their house or apartment. This may include having utilities turned on, assisting with rent, and working with landlords
• Any other duties as assigned.

Required Education and Experience:
Bachelor's Degree preferred in social service, criminal justice, or a related discipline. Proficient in computer skills, ability to work as a team, and have good interpersonal skills.
