The Care Coordinator is responsible for providing patients with appropriate referrals to internal/external resources and agencies, coordinate specialty care for clinic patients, schedule appointments for clinic patients with a provider of a physician, serve as a liaison between the Provider, Registered Nurse (RN), Licensed Vocational Nurse (LVN), and Care Team Supervisor to ensure patients receive appropriate care. Must interact in a positive manner with staff, patients, families, and members of the community.

ESSENTIAL JOB FUNCTIONS:

  • Completes routine reports, messages, and forms
  • Communicates in written and verbal form, using correct grammar
  • Submits all correspondence within the requested time frame
  • Relays all messages as instructed
  • Organizes and files record according to established clinic procedures
  • Files records in chronological and alphabetical order
  • Effectively utilizes office machines to include a computer, fax, and photocopier
  • Collects and distributes mail in a timely manner, and in accordance with established clinic procedures
  • Participates in huddles
  • Schedules appointments
  • Takes messages for the Care Team
  • Follows all policies and procedures
  • Sends authorizations and referrals, completes and documents follow-up on authorizations and referrals to outside agencies, as determined by medical staff to ensure continuity of patient care

SPECIFIC JOB COMPETENCIES: (Referrals)

  • Follows-up on failed appointments via telephone and correspondence for children with special needs.
  • Serves as liaison between clinic, school nurses, regional center workers, public health department and other agencies contributing to the care of clinic patients.
  • Works with several subspecialist physician offices to prepare paperwork and arrange for scheduling of surgeries and pre-op physicals to maximize the provision of care delivered.
  • Assists health department staff with follow-up on CHDP PM-160 Referrals.
  • Assist medical staff in the completion of patient forms (ie. DME’s, IHSS Program, DMV, Medi-Cal Exemption Forms, Etc.)
  • Application for Medical Alert Bracelets.
  • Assists patients of the clinic to call their HMO plans to switch their health plan and/or PCP.
  • Assists medical staff as an interpreter and cultural facilitator for chronically ill clinic patients

EDUCATION AND QUALIFICATION REQUIREMENTS:

  • Medical Assistant Certification required
  • High School Diploma or GED required
  • Must have 2-4 years of medical office experience in an out-patient clinical setting.
  • Must have demonstrated skill in filling, use of copier equipment.
  • Excellent organization and communication skills with attention to detail skills is required.
  • Bilingual in Spanish OR Khmer (strongly preferred)

Work Environment

The noise level of the work environment is usually moderate. Workspace is in an indoor office setting, shared workspace, air-conditioned in the summer and central heat during the winter. Occasionally required to be outdoors for travel, and can be exposed to variable weather conditions.
