Title: QDDP Reports to: Day Service Coordinator
Department: Day Service for Adults
General Summary: Under general supervision, the QDDP:
· utilizes basic behavior principles including visual supports, behavior maps, visual schedules, social stories, and augmentative communication.
· plans and implements daily activities for the program.
· Assists with toileting and personal hygiene for each consumer.
· executes behavior plans as directed by case manager and/or behavior specialist.
· maintains specific knowledge of each consumer’s behavior plan.
· interacts with consumer appropriately and initiates leisure activities throughout the day.
· plans and implements community based outings throughout each week.
· supervises and maintains the safety of all consumers at all times.
· records dates (day, month, and year), all data including PCP(Person Centered Plan) data, behavior data, data for daily activities and all data as needed for reporting procedures as outlined for the Department of Mental Health.
· performs tasks as directed by the Director of Day Services, Day Services Coordinator, and Executive Director as necessary.
· completes training as outlined in The Center for Autism Education policy book.
· effectively completes SCM and uses restraint/transfer techniques taught to intervene with consumers experiencing crisis.
· provides direct supervision of day service direct support staff.
· interviews and hires direct support staff
· demonstrates written and organizational skills at the level that enables adequate completion of job responsibilities.
· Effectively able to stoop, bend, lift up to 20 pounds, kneel, sit on the floor, walk a distance of up to one mile at a time, and run at a brisk pace in order to support a consumer who elopes.
· provides accurate passing of medication, as applicable.
Knowledge, Abilities and Responsibilities Required:
The employee should demonstrate the philosophy and values of The Center for Autism Education, while giving leadership and innovation in the field. A support coordinator should have a Bachelor’s degree in social service or related field (QMDDP qualified), and have experience with MR/DD population.
Adults, parents, and families of The Center of Autism Education will be the population served by this position. Interpersonal Skills will require oral and written communication skills to communicate with staff and families.
Working Conditions:
Work will be performed inside The Center for Autism Education, in a temperature controlled, smoke free environment. Work may also be performed in the form of community outings with the consumer. The knowledge and use of universal precautions is expected as this position may be exposed to air-borne particles or body fluids while working with consumers.

Pay: $38,500.00 - $50,000.00 per year
