The Arc Baltimore is Hiring a Holistic Support Manager - Meaningful Day Near Baltimore, MD

We are actively recruiting for a Holistic Support Manager-Meaningful Day (Seton). The Holistic Support Manager-Meaningful Day will facilitate and encourage communication between a wide variety of internal and external stakeholders, ensuring a truly holistic approach to ensuring people with developmental disabilities to lead fulfilling lives with a sense of belonging, purpose and meaningful relationships.

The Holistic Support Manager will be a liaison between all stakeholders and The Arc Baltimore regarding a person’s services and supports to achieve a person’s self-defined vision for a good life.

Essential Job Functions

  • Coordinate and facilitate the person-centered planning activities with the Person, their Coordinator of Community Services, all members of the person’s self-identified team, and ensure compliance with applicable DDA and other regulations.
  • Initiate person-centered planning activities for people supported and assist them in presenting goals and services at team meetings. Using Charting the Life Course framework, the Holistic Support Manager will be responsible for helping a person develop supports that meet current needs as well as looking forward to help the person achieve their vision for a good life.
  • Identify and communicate strengths and barriers to people supported and possible methods of enhancing strengths and minimizing, utilizing progress notes and/or assessment related tools and/or records.
  • Communicate regularly and respond to inquiries/requests from people supported, family, caregivers, peers,
  • Coordinators of Community Services and all other internal and external stakeholders in a timely manner; when needed work with and connect people served with Management of funds team and/or Benefits Counseling.
  • Train and regularly communicate with direct support professionals on the person-centered plan and appropriate behavioral plan(s), the person’s progress and situational outcomes for people supported.
  • Ensure person(s) served fundamental rights are preserved (COMAR 10.22.04) and satisfactory conclusion to incident reports in a timely manner.
  • Complete appropriate progress reports, iCM documentation, and incident reporting and other organizational/departmental paperwork in required timeframes.
  • Ensure that information in the Person-Centered Plan is current and accurately reflects the preferences and desires of the person supported and the services and supports provided by The Arc Baltimore. Accept and track all Service
  • Authorizations and ensure all services provided have the proper funding. Advocate and escalate issues as necessary to ensure resources are in place in a timely fashion.
  • Attend all meetings and trainings as required.
  • Participate in The Arc Baltimore’s committees and work groups. Assist with The Arc Baltimore’s fundraising and other events.
  • Other duties as assigned.

Education & Experience

  • High School Diploma or GED required. Bachelor’s degree in social work, Psychology, Special Education, or a related field preferred. In lieu of a bachelor’s degree, five years’ experience in the disability field.
  • One year of case management and supervision experience in the disability field is required for this position. In lieu of such experience, five years’ experience as a Direct Support Professional may be substituted.
  • Excellent oral and written communication skills.
  • Proficiency in Microsoft Office and other technology tools is required.
  • Valid driver’s license and be eligible to drive agency vehicles.

  • Position requires frequent standing and walking.
  • Must be available to work a flexible schedule as dictated by the needs of the agency.
