POSITION TITLE: Housing Case Manager

DEPARTMENT: Reentry

REPORTS TO: Associate Director

STATUS: Full TIME

GRANT FUNDED: This position is grant-funded and dependent on the grant availability.

Teamwork Englewood (TWE) Teamwork Englewood is a community-based organization that

provides programming and planning for Englewood residents. The organization's mission is to improve the quality of life of the residents and stakeholders of Englewood by facilitating

economic, educational, and social opportunities. Teamwork Englewood is a growing not-for-profit organization that serves the Greater Englewood community. The organization offers several programs, from services to returning citizens, youth development programming, workforce development services, and parent programming. Teamwork Englewood also provides community programming and services for Englewood residents, as well as community planning and engagement through the Englewood Quality of Life Plan.

Teamwork Englewood Housing Support Program Overview: Teamwork Englewood and All Chicago are partnering to provide short-term rental assistance and services for individuals with a criminal background. The goals are to help people obtain housing quickly, increase self-sufficiency, and stay housed. It is offered without preconditions (such as employment, income, absence of a criminal record, or sobriety), and the resources and services are typically tailored to the person's needs.

The Housing Case Manager will provide intensive case management for participants placed in temporary apartments seeking permanent housing. The Case Manager will assist individuals with identifying barriers to housing stability and engage in goal planning that aims to link individuals to the resources and skills they need to achieve long-term housing stability. This person is responsible for fostering a safe and trauma-informed environment and must have experience working with mental illnesses and substance abuse disorders. Most importantly, the case manager should have a passion for working with individuals experiencing homelessness and those returning from prison in our community. This position requires significant record-keeping and documentation of services, which must be completed promptly, as well as data entry.

Responsibilities

  • Intensive Case Management: Conduct initial assessment and screening of all potential clients to determine eligibility.
  • Service Planning: Utilize diversion skills to help those at risk of homelessness to identify immediate alternate housing arrangements, connect them with needed resources, and identify strengths and personal resources.
  • Utilize all relevant resources (outside agencies, social support, budget assistance, etc.) to avoid eviction as much as possible.
  • Identify suitable landlords/units.
  • Maintain positive relationships with landlords/property managers from the Housing Resource Listing.
  • Develop and implement Transition Plans for all households in Transitional Housing programs (if applicable)
  • Complete the participant's data entry/exits within the Homeless Management Information System.
  • Establish partnerships with other agencies and community resources
  • Determine eligibility for emergency rental assistance/homeless prevention funds
  • Produce monthly, quarterly, and annual reports.
  • Interprets and implements the right to privacy, accuracy, and retention of criminal background records and course repetition requests. Obtain client consent for the release of said information before disclosure.
  • Work with the Job Developer to develop and assign employment opportunities to reentry clients and support their retention through ongoing communication and warp-around supports when available.
  • Must maintain a positive rapport with the clients to ensure successful program outcomes.
  • Adhere to all guidelines related to confidentiality of files and records
  • Must work independently and collaboratively with a team approach while maintaining case management for assigned caseload.
  • Attend staff meetings, training, and supervisory meetings as instructed by the supervisor.
  • Maintain current knowledge base by participating in educational or training opportunities, reading relevant publications, and maintaining professional networks.
  • Perform other duties as assigned by the supervisor

Qualifications

  • Able to work independently, submits accurate documentation, and seeks guidance when needed
  • Accepts feedback, takes ownership of responsibilities, and identifies areas of growth
  • Communicates clearly (both written and verbally), seeks solutions, and demonstrates flexibility.
  • A degree in a related social service field is preferred, or 5 years of relevant experience instead of a degree.
  • 1 – 2 years of experience working with individuals experiencing homelessness and/or individuals with a history of trauma is required.
  • Possess and maintain a valid license and safe and reliable transportation to travel to various sites as required (mileage reimbursement is available).
  • Possess and maintain adequate Liability Insurance for the vehicle.
  • Strong computer skills (Microsoft Office, database software)
