St. Ann's is Hiring a Life Enrichment Coordinator Near Grand Rapids, MI

Job Summary:

The primary purpose of the Life Enrichment Coordinator is to assist in the planning, implementation and evaluation of recreational, social, intellectual, emotional and spiritual programs, in accordance with the resident’s assessment and care plan, and as may be directed by your supervisor.
Essential Functions:
  • Duties/Responsibilities: Responsible for initial introduction with new residents to determine their likes/dislikes and wants/needs to ensure the transition to St. Ann’s is smooth and to ensure success in their new environment. Identify and implement resident activity programming to meet resident needs, promote participation and keep residents engaged. Develop a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident and the goals to be accomplished for each problem/need identified. Develop preliminary and comprehensive assessments of the activity needs of each resident. Review and revise care plans and assessments as necessary, but at least quarterly. Observe resident attendance, mood, behavior, and engagement so that facility activities and resident progress are evaluated, by noting, reporting, and charting resident behavior. Participate in discharge planning, development and implementation of activity care plans and resident assessments. Coordinate resident council meetings. Assist in arranging transportation for outings when necessary.
  • Administration: Lead daily staff meetings to help identify and correct problem areas and/or improve services. Assist with any scheduling challenges that may arise. Assist with training staff on person-centered activities including dementia specific life enrichment. Assist in developing, implementing, and maintaining an ongoing quality assurance program for the life enrichment department. Ensure that all charted activity progress notes are informative and descriptive of the services provided and indicate the resident’s response to the service. Perform administrative requirements, such as completing necessary forms, reports, etc., and submitting such to the Director of Resident Life as required.
  • Teamwork: Participate in facility surveys (inspections) made by authorized government agencies as necessary or as may be directed. Participate in community planning related to the interests of St. Ann’s Home and the services and needs of the resident and family. Coordinate activities with other departments as necessary. Involve the resident/family in planning activity programs and planning objectives and goals for the resident, when possible. Participate in Care Conferences for Skilled Nursing Residents. Serve on, participate in, and attend various committees of St. Ann’s Home as appointed.
  • Communication: Interview residents or family members to obtain activity information.Refer resident/families to appropriate social service personnel when St. Ann’s Home does not provide the services or needs of the resident. Interview residents or family members to obtain activity information.
  • Flexibility: The ability to shift gears and implement new topics and processes quickly to address emerging resident needs. The work schedule may require conformity to resident needs and outcomes.
  • Other duties as assigned to meet standards and organizational goals.

Education and Experience Requirements:

  • Must possess, as a minimum, bachelor degree in recreational therapy or related field.
  • Minimum of two years’ experience in a long-term care environment.
  • Experience working with Dementia
  • Ability to work in a fast-paced environment with multiple competing demands for time and attention.
  • Works well in team environment
  • The ability to locate, track, organize and summarize information.
  • Able and willing to coach and train others.
