We provide comprehensive infusion therapy services in the home setting or in our outpatient ambulatory infusion suites. Our clinical team of nurses and pharmacists work closely with physicians to develop a plan of care based on the patient's therapy. From pharmacokinetic monitoring to reviewing medication profiles, our pharmacists work closely with patients and their physicians to ensure high-quality care.

Role Description

This is a part-time, on-site, role located in Tempe, AZ. As a Home Infusion Pharmacist at Sisu Healthcare Solutions, you will play a crucial role in providing excellent clinical service to our infusion therapy patients. Your day-to-day tasks will include assessing medication profiles, preparing and dispensing medications, and collaborating with the clinical team to develop and implement patient care plans. You will also be responsible for ensuring compliance with regulations and guidelines, as well as providing patient education and support.

Qualifications

• Pharmacy degree from an accredited institution
• Current and unrestricted Pharmacist license in the state of Arizona
• Knowledge of infusion therapies and medications
• Experience in a home infusion or hospital pharmacy setting
• Strong attention to detail and accuracy
• Excellent communication and interpersonal skills
• Ability to work effectively in a multidisciplinary team
• Knowledge of pharmacy laws, regulations, and standards of practice
• Proficiency in pharmacy software and computer systems
