 Our Rehab Liaison will 

• Promote the programs and services of PRN Home Health and Therapy 
• Act as patient liaison in coordinating transition to home health services
• Understand the basic principles of public relations and/or marketing
• Act as company liaison in identifying and educating patients who would benefit from home health services 
• Love working in a fast paced environment with a supportive team
• Be great communicator with customers, patients and team members

Qualifications: 

• Licensed in Wisconsin as a Physical Therapist Assistant or Occupational Therapy Assistant
• Excellent verbal and written communication skills
• Ability to work individually or in a team environment; Self-directing, able to work with little direct supervision
• Good judgement and discretion when dealing with sensitive topics
• Able to interact with many people and multiple interruptions while staying on task
• Maintains a valid drivers license and auto insurance
