Passionate care Hospice is looking for a flexible Hospice Social Worker, with LMSW AND HAS HOSPICE EXPERIENCE. WILLING TO SEE PATIENTS ON A PRN /PART TIME. MUST ATTEND IDG MEETINGS

QUALIFICATION.

Masters of Social Work from an accredited school of social work

Holds current unencumbered license of Social Work in the state practicing in

Minimum one (1) year experience as a social worker in Hospice/ Home Care /Community /Health long term care or medical surgical/acute care setting

Must be able to work FLEXIBLE hours and travel between offices, facilities, etc
Must have ability to handle stress associated with ministering to patients/residents and their loved ones who experience pain, suffering, death and grief

Responsibilities :

• Able to visit hospice patients at their residence. To assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment .

Assist patients/CG in making funeral arrangements , obtaining Advanced Directives , Initiating Power of Attorney and other needs.Arrange for patients placement , to include facility transfers, respite care etc.Assess the relationship of the patient's medical and nursing requirements to the patient's home situation, financial resources, and availability of community resources, to include funeral arrangements if neededFull descriptionPassionate care Hospice is GROWING Hospice . Compassionate Licensed Medical Social Worker (MSW) to provide hospice social services in Metro -Atlanta area. Someone who is available to attend our Interdisciplinary meetings as required.
Our Social Worker (LMSW) provides social service counseling and guidance to facilitate the maximum functioning and coping capacity to each patient and caregiver/family member.

Job Types: Part-time, Per diem

Pay: $30.00 - $45.00 per hour
