245D Designated Manager/Coordinator

The 245d Designated Manager supports services in Minnesota

Purpose Statement:

The role of the 245d Designated Manager is to support and train Residential Supervisors and Program Managers to provide the highest quality of care to the individuals we serve, develop new innovative supports, identify areas for programmatic development/effectiveness/efficiency, foster a positive culture focused on goal outcomes among interdisciplinary teams and agencies and encourage transition to the most independent and integrated life for those we serve.

Hourly wage $19 - $22 /hour, Based on Qualifications and Experience.

Job Duties and Responsibilities: (Essential Functions)

  • Train and support direct reports in your department and all RSs.

  • Person Served Team Communication and Involvement
  • Educate, train, and support supervisors as it relates to completion and content of 245D paperwork as well as oversee and update programming documents:
  • Oversee the development and implementation of Rights Restriction
  • Develop program protocols as necessary and specialized individual programming to meet individual need
  • Annuals: Assist RS’s with content in annual Progress Report and Recommendations (and when applicable, Semi Annuals). Attend annual meetings when a person is having specific issues that make it difficult to serve or when a person is doing exceptionally well and ready to transition to a more independent setting.
  • Person-Centered Planning
  • Absences/Crisis/Discharge; Suspension of service for all absences; 6910/6790/change orders
  • Community Integration/ Purposeful Day Development and Implementation - natural supports, non-resident supports, community supports
  • Hiring DSPs, interviewing, HR duties

Designated Manager Job Duties and Responsibilities

  • Maintaining a current understanding of the licensing requirements sufficient to ensure compliance throughout the program as identified in 245A.04, subdivision 1, paragraph (e), and when applicable, 256B.04, subdivision 21, paragraph (b).
  • Ensuring the duties of the Designated Coordinator are fulfilled according to 245D.09, subdivision 2.
  • Ensuring the program implements corrective action identified as necessary by the program following review of incident and emergency reports according to 245D.11, subdivision 2, clause (7) including that an internal review has been completed for situations that require one.
  • Evaluation of satisfaction of persons served and/or legal representatives and the case managers, with the service delivery and progress towards accomplishing outcomes identified in 245D.07 and 245D.071, and ensuring and protecting each person’s rights as identified in 245D.04.
  • Ensuring staff competency requirements are met according to 245D.09, subdivision 3 and ensuring staff orientation and training is provided according to the requirements in 245D.09, subdivisions 4, 4a, and 5.
  • Ensuring corrective action is taken when ordered by the commissioner (DHS) and that the terms and condition of the license and any variances are met.
  • Evaluating the information identified in the previous six points to develop, document, and implement ongoing program improvements.

Required Skills and Experience (Minimum requirements in terms of educational background, work experience, licenses/certifications, or other knowledge, skills, and abilities).

  • (1) a bachelor's degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older;
  • (2) an associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older;
  • (3) a diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or
  • (4) a minimum of 50 hours of education and training related to human services and disabilities; and
  • (5) four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3)
  • (6) a minimum of three years of supervisory level experience in a program providing direct support services to persons with disabilities or persons age 65 and older

Additional Skills and Experience (Preferred or helpful)

  • 245D or healthcare experience
