New Directions Northwest is Hiring a Service Coordinator / Eligibility Specialist Near Baker, OR

General Purpose: Provide service coordination to assigned clients in the Developmental Disabilities (DD) program as outlined by Oregon Administrative Rules and New Directions Northwest, Inc. Policies and Procedures.

Supervision Received: Works under the direction of the DD Program Manager. This position is not required to supervise other employees.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Provide case management services to all clients. Services are provided in a variety of community settings, with consideration to client preference.
  • Responsible for maintenance and timeliness of all records, progress notes, and other required documentation.
  • Assist clients in acquiring services and resources needed to achieve goals and maintain independence within the community.
  • Coordinates with other Community Partners to develop, provide, and maintain a rich array of services and resources for clients.
  • Provide transportation for clients when required.
  • Attend local, regional, and/or State meetings as scheduled.
  • Act independently within the community and promote a positive image of the agency.
  • Participate in all team and agency meetings as scheduled.
  • Adhere to all Developmental Disabilities program policies and procedures.
  • Keep updated on all ISP meetings and established objectives.
  • Meet all billable service contract requirements during the month, unless otherwise arranged through the CEO.
  • Act as a broker for services in order to ensure clients meet necessary contacts (e.g. psychiatrist, medical doctor, SSA, etc.) as required.
  • Ensure that all Waivers, Variances, and CPMS forms are submitted within the required timeframes and kept current if required.
  • Participates in joint case planning and coordination with internal multi-disciplinary team and agency psychiatrists as needed for client services.
  • Remain updated on all rules and OAR's pending to DD services.
  • Ensure that all contractors and providers, that New Directions Northwest is required to monitor, adhere to relevant OAR directives.
  • Coordinate regularly with the Mental Health Services program on clients who receive mutual services.
  • Act as a resource for on-call staff in consultation or involvement in reference to DD clients.

PERIPHERAL DUTIES

There may also be other duties assigned by the Program Director or his/her designee.

MINIMUM REQUIREMENTS / QUALIFICATIONS

Education and Experience:

  • A minimum of a Bachelor's Degree or equivalent experience. Experience with DD and co-occurring disorders preferred.

Other Skills and Abilities:

  • Must possess knowledge of MED case management standards and the ability to implement them with the community. Maintain familiarity with OAR's pertaining to the MED system. Must have the ability to effectively communicate both verbally and in writing and have the ability to work independently and complete all responsibilities in a thorough and timely fashion. Possess the ability to represent the interest of the client and the agency in a favorable light to the community.
  • Must be computer literate and have the ability to type and utilize word processing and other software programs/systems.

Valid Oregon Driver's License

  • Must possess a valid driver's license and be insurable for client service purposes and for travel between business offices and to attend required meetings and/or trainings. Employee may also be required to provide client transportation on occasion. Utilize company-owned vehicles when available. Must pass all criminal history check requirements as required by ORS 181.536-181.537 and in accordance with OAR 410-007-0200 - 410-007-0380.

LICENSURE AND CERTIFICATIONS

  • Continuing education and previous training in social service-related fields helpful.

Applicants should be able to work collegially with all other professions and should demonstrate tolerance of those with different opinions and/or attitudes.
