Minimum Requirements:

• Current state license as a Registered Nurse
• At least one year of nursing experience is needed
• CPR/BLS Certified

Essential Job Functions/Responsibilities

The registered nurse plans, organizes and directs home care services. The registered nurse builds from the resources of the community to help plan and direct services to meet the needs of individuals and families within their homes and communities. From developing care plans, counseling the patient and family of nursing and related needs, to identifying discharge planning, the registered nurse will provide services within the scope of nursing. Under the clinical supervisor, the registered nurse will assume responsibility to coordinate patient care for an assigned case load and communicate with community health related persons to coordinate care plan.

License/Certification:

• Current RN License (ADN/RN/BSN)
• Current BLS/CPR Certification
• Current CA Driver's License
