JOB DESCRIPTION:

Living with Purpose of Missouri is seeking dedicated indivuduals to join our team as Companion Home. In this role, you will be responsible for providing essential support to service recipients based on their Individual Support Plan. The primary focus will be on assisting individuals with self-care routines, self-help tasks, and promoting overall health and well-being.

Self-Care and Self-Help Tasks:

· Assist service recipients in completing self-care routines and self-help tasks as outlined in their Individual Support Plan.

Health Maintenance:

· Follow prescribed plans of care, including nutrition plans, health care plans, and exercise regimes.

· Administer prescribed medications as needed.

Community Access and Transportation:

· Provide support for service recipients to access the community, including shopping, leisure activities, and health-related appointments.

· Utilize personal vehicles to ensure safe and reliable transportation.

Home and Vehicle Maintenance:

· Ensure the service recipient’s home is clean, safe, and free of maintenance concerns.

· Maintain the personal vehicle used for transportation, ensuring it is safe and in good working order.

Relationship Development:

· Support service recipients in developing and maintaining relationships of their choice.

Person-Specific Plans:

· Assist in the development of person-specific plans and diligently implement all plans.

Documentation:

· Complete required documentation, including Staff Daily Communication Notes, Reportable Incident forms, and Medication Variance forms, in a thorough manner.

· Submit all required documentation to appropriate personnel in a timely manner.

Adherence to Schedule:

· Work the assigned schedule in accordance with living with Purpose of Missouri Policy and Procedures.

Team Collaboration:

· Work collaboratively with all Living with Purpose of Missouri personnel, promoting effective communication, conflict resolution, and positive interpersonal interactions.

Training and Meetings:

· Attend Team Meetings and training sessions as required by the organization.

Mission Adherence:

· Perform all job functions in a manner that exemplifies Living with Purpose of Missouri’s mission and philosophy of service delivery.

Other Functions:

· Complete other duties as requested by supervisor and/or Administrator.

REQUIREMENTS:

Age 21 and over. High school diploma or equivalent.

· Live-In Companion Home

· Valid driver’s license

· Use of vehicle during your entire shift

· Proof of auto insurance

· Clean driving record and personal background check

· Drug screening required

· Experience providing personal care as a health aide

Job Types: Full-time, Part-time

Pay: $16.00 per hour
