Social Worker for the 2024-2025 School Year
ORGANIZATION OVERVIEW

Legacy Charter School is a high-performing public charter school holistically educating 500 scholars in pre-k through 8th grade in Chicago’s North Lawndale neighborhood. Our mission is to create an outstanding center for teaching and learning for children and their families that will recognize and nurture the full potential of every child, provide a foundation for a college education, and educate our scholars to be creative and critical thinkers and responsible citizens in school, at home, and in the broader community.

POSITION SUMMARY

As a part of the school support team, the School Social Worker will work collaboratively with the rest of the scholar support team as well as the staff to assist in the operational functions of the school. The School Social Worker will help provide a safe, comfortable, and positive learning environment for all scholars to be able to reach their goals. The School Social Worker will also be able to collaborate and plan with the special education teachers, case manager, and general education teachers through consultative services as well as through accommodations and modifications.

The social worker must have a proven track record of working with families and community agencies to alleviate adverse conditions in order to promote social, emotional, and academic success for all.

PRIMARY RESPONSIBILITIES:
DIRECT SERVICE WITH SCHOLARS
  • Provide individual social work services to scholars who face challenges that impact learning
  • Plan and facilitate SEL small groups, skill training groups, and classroom sessions
  • Conduct behavioral assessments and assist in implementing and monitoring individualized scholar support plans
  • Provide SW minutes to scholars as required by IEPs
  • Plan and facilitate individual and small group sessions
  • Serve as a liaison between the school and outside medical and social service providers to streamline supports
  • Complete scholar risk assessments as needed
  • Provide crisis intervention services as needed - Serve as a liaison with community agencies and assist in fostering communication between schools, parents/guardians, and community leaders
  • Locate and mobilize community resources to support children and families' success in and outside of school

DIRECT SERVICE WITH FAMILIES

  • Communicate with families to support scholars and help them become engaged advocates in their children’s lives; assist families in supporting their child’s individual learning needs
  • Collaborate with community agencies to assist families in accessing outside services such as housing, medical, and legal services

DIRECT SUPPORT WITH TEACHERS

  • Observe classes to provide suggestions for social-emotional learning
  • Participate as a member of the MTSS and other school-based teams to develop interventions for promoting scholars’ academic success
  • Consult and collaborate with school personnel in implementing strategies to promote scholar learning - Provide staff consultation on behavioral-emotional-environmental issues affecting scholars’ participation in the learning process

COMPLIANCE AND COORDINATION OF THE IEP PROCESS

  • Create measurable goals for scholars who receive support or services through an IEP and uses a variety of assessment tools to evaluate scholars
  • Is familiar with and is able to support in conducting Functional Behavior Assessments and creating and monitoring scholar Behavior Improvement Plans
  • Observe and assess scholars as part of the special education evaluation and reassessment process
  • Enter ongoing social work minutes and notes into the compliance system
  • Participate in special education meetings
  • Maintain required clinical records and submit appropriate documents for reports

  • Is familiar with Special Education Law
  • Is experienced in implementing and supporting Behavior MTSS (Multi-Tiered System of Supports) processes
  • Is able to work with DCFS, the STLS program, and other support services
  • Can coordinate multiple services for scholars and their families

REQUIREMENTS

  • Master’s degree in Social Work from an accredited college or university
  • PEL with social work endorsement k-12, school service personnel certificate (k-12)
  • Experience supporting children and families in an urban school environment
