Heartford Hospice is a premier hospice organization which provides expertise, compassion, and care for our patients and their families as they face terminal illness. Our team of professionals helps improve quality of life by addressing the physical, emotional and spiritual needs of our patients. The Certified Home Health Aide (CHHA) is an important member of the care team who performs various personal care services to meet the patient’s personal needs while promoting the patient’s comfort, safety and peace of mind. The CHHA is responsible for observing patients, reporting these observations, and documenting observations and care performed. The CHHA works with the Hospice team and the patient’s caregivers to ensure excellence in care that is delivered in accordance with the patient’s wishes.
