"Join our team as Team Leader in Training/Team Leader working in the rewarding field of Adults with Disabilities. Hazel Grace Homes, LLC is an award winning HPC provider that offers community supports to individuals with intellectual and developmental disabilities. We're looking for an energetic, caring, self-motivated, and well-organized individual with tech-savviness and excellent communication skills.

They will oversee the training, staffing and operation of two of our sites and will be extremely instrumental in building a team of direct support professionals who provide exceptional services. The starting wage of this full-time position for our Team Leader in Training position is $17 per hour/ Team Leader is $18 per hour. All new hires will start as Team Leader in Training until successful completion of our Team Leader Series training. This position may have a varied schedule may require working nights and weekends.

Job Duties:

• Helping to build and support a well running team of direct support professionals to ensure high quality services.
• 24/7 on call responsibilities for our homes with every other weekend on-call rotation
• Be familiar with and assist with all operations, including staffing, payroll, and financial management of each home.
• Provide direct support to the individuals that we
• Provide training to direct support professional on individual support plans (ISP) and ensure these plans are followed.
• Provide oversight to employee training, staffing and certifications per the agency’s policies and procedures and DODD regulations.
• Effectively address all policy, procedure and disciplinary related questions and ensure timely completion of performance evaluations.

Job Requirements:

• 21 years of age
• A working vehicle which may be used for transportation and during the course of work day.
• Valid auto insurance for that vehicle
• A valid Ohio Driver’s License with 4 points or less on your BMV record
• A high school diploma or equivalent
• An ability to pass background checks per Ohio DODD
