FIVE ACRES - THE BOYS' AND GIRLS' AID SOCIETY OF LOS ANGELES COUNTY is Hiring a Case Manager - American Sign Language (ASL) Near Pasadena, CA

Summary

Must be fluent in American Sign Language (ASL). Responsible for working with the treatment team to ensure the provision of case management services for children and families at Five Acres in accordance with policies and the clients' treatment plans. 

Essential Duties

  • Attends regularly scheduled treatment planning meetings with clinicians and treatment team members to determine the needs of each child for case management services; coordinates case management services to ensure the needed services are provided, and necessary documentation is completed.
  • Complies with productivity standards of the Agency and maintains an appropriate caseload to reach the budgeted daily expectation of billable hours. (Refer to Productivity Policy)
  • Submits timely documentation (progress notes, assessments, treatment plans, notes to chart) as outlined in policy and measured via productivity reports. (Refer to Progress/Case Note Submission Policy) 
  • Establishes and maintains relationships with school districts and community organizations; refers clients to appropriate community services; coordinates contacts and visits; escorts clients to referral appointments when applicable.
  • Provides service plan updates; monitors service plan activities and follows-up with referral agencies regarding client progress to ensure client outcomes.
  • Makes necessary contacts with treatment team members (e.g., attorneys, CSW, family members, agency volunteers, CASA Workers, auxiliary treatment programs, school personnel, etc.) as assigned by the clinician; documents all contacts in the child's file.
  • Gathers data and assists the team in data collection to track service outcomes; completes monthly reports to capture program services.
  • Develops and conducts parent/youth workshops as needed by school and neighboring communities.
  • Attends SSTs, IEPs, psychiatric visits, and community events to act as a liaison on behalf of the treatment team of Five Acres as appropriate.
  • Meets with clients at minimum on a monthly basis to provide education and support.
  • Qualifications

    • Bachelor’s degree in social work, psychology, Marriage and Family Counseling or closely related mental health field.
    • Proficient in American Sign Language (ASL).
    • Previous work experience working with severely emotionally disturbed children and their families.
    • Effective oral/written communication skills; bi-lingual skills in Spanish/English preferred.
    • Ability to be flexible and work in a changing environment.
    • Computer skills sufficient to meet production demands of the job including Microsoft Office and TIER/WELLIGENT software, and skill in maintaining detailed, accurate and complete records.
    • Must have a valid California Driver License, proof of automobile insurance and meet the Agency's safe driver guidelines.
    • Ability to work varied hours.
    • Sensitivity to the different cultures represented among the clients, families and staff.
    • A basic understanding of age-appropriate behavior for children/adolescents.
    • Knowledge of emergency procedures of the Agency.
    • Ability to model positive behaviors and Agency values.
    • Ability to work effectively as part of a team.
    • Strong problem-solvng skills.

    Compensation Range: $19.38/hr - $28.11/hr
