Corvallis Caring Place (CCP), a small non-profit Assisted Living Facility in Corvallis, Oregon, is seeking an experienced Registered Nurse to join our team as Health Services Director (HSD). The HSD is responsible for overseeing the Health Services Department and care of residents, including evaluations, service planning, and care coordination. The HSD manages the Health Services team through supervision, education, and support.

Duties and Responsibilities:

· Provides nursing expertise by regularly evaluating resident needs and changes of health condition. Assists in writing service plans, communicates information to direct care staff and families, and guides staff in direct care delivery.

· Communicates with external providers about resident illness/health and ensures that provider orders are followed.

· Interacts with families, physician office staff, pharmacists, home-health practitioners, Hospice staff, and others to ensure resident needs are met; promotes good facility public relations during interactions.

· Provides on-going training and guidance for Medication Technicians. Delegates and assigns tasks as appropriate. Oversees accuracy in ordering and administering medications and treatments.

· Promotes teamwork among staff to provide excellent resident services.

· Completes MAR audits and monitors medication room to assure organization and cleanliness.

· Assists with tracking and recording incident reports. Assures incident investigations are completed.

· Ensures compliance with Oregon Assisted Living regulations as they pertain to medical and health services functions.

Education and Experience

· Must be a Licensed Registered Nurse in Oregon

· Experience in long-term care or clinical nursing practice preferred

· Demonstrate strong interpersonal communication skills

· Exhibit good time management and organization skills

· Possess the ability to handle complex issues

· Proficient in Microsoft programs (Word, Excel and Outlook)

· Express compassion for residents, staff and guests
