BRC Mental Health Services/Boise Residential Care (BRC) is hiring a full time Case Manager. BRC is a housing provider and a mental health agency creating a new type of housing and mental health clinical services all in one program for those diagnosed with a serious and persistent mental illness (SPMI). You will work half the time remote but will need to go to the houses, agency various times throughout the week.

Duties

• Works with and links clients to access the resources needed to create an environment that supports recovery, growth, and stability.
• Oversees and documents the client’s needs for services for individuals through comprehensive assessment, service planning, referral, and monitoring of the client’s service needs/activities.
• Oversee and assist the coordinators with auditing the client’s schedule, appointments and records.
• Assists the coordinators with intakes and admissions.
• Assists the Coordinators with auditing and updating Treatment Plans, SRF's, CDA's, Wellness Assessments, Functional Needs Assessments, Service Plans & ADL’s.
• Will audit and oversee payee services including auditing bank accounts and budgets.
• Provide services to clients who are unable to navigate the service system on their own, and do not have the assistance of other natural supports or resources to help them access needed services.
• Case management services do not include the direct delivery of services and cannot be provided by a behavioral health network provider when the client is already receiving the service from another Medicaid provider.
• Helps the client who is seeking services, to learn about or gain access to behavioral health, medical and social services and providers, until the client is capable of accessing these services independently.

Requirements

• Be able to pass a background check with Health and Welfare
• Bachelor’s degree

Preferred qualifications

• Knowledge or experience in Assisted Living
• Bachelor’s degree or experience in Appointments or Scheduling
• Experience in Behavioral or Mental Health
