Job Title: Case Manager
Job Category
: Full-Time is Exempt
Definition:Under the supervision of the Clinical Director, the primary responsibility of this position is the coordination and provision of services to persons affected by brain injury. These services are provided using a model of community-based case management.
Distinguishing Characteristics:This position requires the individual to practice community-based case management principles within the context of the policies and procedures governing Brain Injury Connections of the Shenandoah Valley. The individual will identify resources in the community for persons affected by brain injury and develop referral sources. The position assumes the ability to work with minimal supervision.
The Executive Director and Clinical Director may assign other duties.
Examples of Duties:Guided by the mission and values of the organization, a full-time Case Manager shall perform, at a minimum, but not limited to the following:
  • Maintain a caseload of at least 25 active cases and 5 follow-along cases based on a 40-hour workweek. The caseload is prorated for full-time between 30-39 hours a week, or part-time.
  • Complete and maintain all documentation designated in various policies and procedures.
  • Perform a comprehensive intake evaluation including the completion of a thorough client assessment, which includes the client’s strengths and weaknesses, family and community support systems, a needs assessment that covers all areas described in our policies and procedures manuals, and the assessment of existing resources presently utilized by the client/family.
  • Support clients/caregivers in developing their own goals and plans utilizing a person-centered approach and based on the above needs assessment.
  • Maintain regular contact (minimum 30-day intervals) with clients/caregivers offering them support and guidance and assisting them in maximizing their participation in the goal attainment process.
  • Identify, coordinate and monitor goal-directed community services to meet the client’s needs and wants.
  • Maintain and document client information in a web-based database.
  • Obtain information and make referrals for appropriate services;
  • Obtain all necessary records from outside sources to assure a thorough evaluation and eligibility for services.
  • Establish a therapeutic relationship with clients and their families/caregivers.
  • Assess assistive technology needs and seek appropriate resources to meet needs.
  • Obtain appropriate releases and understand the laws regarding confidentiality and uphold these laws.
  • Communicate effectively with clients, their families, and other support systems and professionals.
  • Create and facilitate support teams to include referrals, scheduling of meetings and delegation of duties.
  • Advocate on behalf of clients and their caregivers.
  • Clearly articulate clients’/families’ needs and appropriateness for services to the Triage Team, as needed.
  • Effectively negotiate for services for clients in their communities in a professional manner.
  • Travel and provide services in client’s home and community.
  • Expend resources within the client’s funding budget and seek approval of expenditures from the Clinical Director or Executive Director.
  • Participate in public awareness and community outreach activities, such as developing referral sources, attending health fairs, providing in-service training, attending fundraising events, coalition building and developing and/or facilitating support groups.
  • Participate in professional development activities and stay abreast of current/new information in the field.
  • Complete other duties as assigned by Clinical Director.
Knowledge Base:
  • Brain injury and its effects on children, adolescents and adults, as well as their families and caregivers;
  • Knowledge of case management practices, supportive counseling techniques, medical terminology, and community agencies/resources.
Job Requirements:
  • Basic computer skills; 
  • Valid driver’s license; 
  • Access to a personal vehicle; 
  • Physical mobility; ability to perform math calculations; 
  • Complex problem-solving and reasoning. 
  • Must be able to read, write and proofread at a professional level. 
  • A minimum of a Bachelor’s Degree is required. 
  • One year experience in case management and/or professional licensure preferred. 
  • Masters's Degree preferred.
  • Certification or a 24-month action plan for acquisition as a brain injury professional certification required.
