Housing Case Manager

Position Summary

To assist in collaboration with the City of Winston-Salem to administer the Rapid Re-Housing and Permanent Supportive Housing programs for HIV individuals who experience homelessness. The goals for both programs are to identify those individuals who are chronically homeless; provide financial assistance to secure stable housing through interactive case management and maintain monthly contact with these individuals to ensure ongoing stability. Additionally, this position will collaborate with colleagues at Positive Wellness Alliance to further the mission of the organization.

Essential duties:

  • Assist in transitioning HIV homeless individuals from a chronic homeless situation into stable housing and consistent medical care and treatment adherence through PSH or Rapid Rehousing Program
  • Make referrals to HOPWA program, as appropriate
  • Utilize data systems such as Excel, CAREWare, HMIS to record data
  • Collaborate with the Coordinated Intake Center through the Continuum of Care, Operating Cabinet, and HMIS transitional trainings
  • Attend meetings required by the City of Winston-Salem and attend any and all meetings with different groups through the City of Winston Salem (i.e., case management meeting) that work.

Client activities:

  • Meet with and/or contact individuals at least monthly or more if necessary via client care calls
  • Connect HIV homeless individuals to medical case management, as appropriate
  • Coordinate with PWA and external case managers/agencies for life skills classes covering topics such as budgeting, job search and interviewing, search for housing, being a responsible tenant, anger management, and the like.

Reporting:

  • Reports to Supportive Services Director
  • Submit timely and accurate financial requests for individuals to assist in preventing homelessness
  • Submit timely and accurate weekly/monthly time management reports to City of Winston Salem and Supportive Services Director.
  • Assist with monthly reporting required by City of Winston-Salem
  • Provide presentations and data on homelessness and HIV persons experiencing homelessness in the agency’s regional service area.
  • Maintain current information about community resources for persons experiencing homelessness

Additional duties:

  • Build strong relationships with organizations that offer emergency financial assistance
  • When applicable, act as an emissary of PWA when representing the agency in meetings or in the capacity of performing the job.
  • Attend PWA staff meetings
  • Assist with support groups
  • Maintain strict adherence to professional codes of ethics that govern the service delivery to clients and their families
  • Participate at a high level at all PWA fundraising and outreach events.

Work Environment:
This position operates in a professional office environment and in the field. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.

Fieldwork requires driving, walking, and meeting individuals wherever they are located.

The position also requires interaction within the community, including communicating in person with the Board of Directors, potential donors, volunteers, and the public.

While performing the duties of this position, the employee is regularly required to use all forms of communication (oral, auditory, and visual) effectively. The employee is occasionally required to travel to other locations.

Ability to bend and lift 35 pounds.

Travel:
Due to PWA having various office locations, travel from home to workstations is not reimbursable. This position may require inner-city travel for work-related services.

Required Education and Experience

  • Bachelor’s degree in social work, human services, or related field
  • Strong written and oral communication skills
  • Experience in administrative/office work
  • Experience in working with diverse populations and proven work in culturally aware environments
  • Computer skills
  • Excellent interpersonal skills

Preferred Education and Experience

  • Master’s degree in social work, human services, or related field
  • Previous work experience in community health or human service agency
  • Experience in marketing, housing, and mental health
  • Experience with Permanent Supportive Housing/Rapid Rehousing.

Work Authorization / Security Clearance

  • Positive Wellness Alliance hires U.S. citizens, permanent residents, asylees, refugees, and temporary residents. Temporary residence does not include those with non-immigrant work authorization (F, J, H, or L visas), such as students in practical training status. Exceptions to these requirements will be determined based on the shortage of qualified candidates with a particular skill.
  • Positive Wellness Alliance will require proof of work authorization.
  • Any offer of employment is conditioned upon the successful completion of a background investigation, which may include Financial, Criminal, Sex Offender, and Substance Abuse screening.
