Housing Case Manager
Position Summary
To assist in collaboration with the City of Winston-Salem to administer the Rapid Re-Housing and Permanent Supportive Housing programs for HIV individuals who experience homelessness. The goals for both programs are to identify those individuals who are chronically homeless; provide financial assistance to secure stable housing through interactive case management and maintain monthly contact with these individuals to ensure ongoing stability. Additionally, this position will collaborate with colleagues at Positive Wellness Alliance to further the mission of the organization.

This position operates in a professional office environment and in the field. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.
Fieldwork requires driving, walking, and meeting individuals wherever they are located.
The position also requires interaction within the community, including communicating in person with the Board of Directors, potential donors, volunteers, and the public.

While performing the duties of this position, the employee is regularly required to use all forms of communication (oral, auditory, and visual) effectively. The employee frequently is required to stand and walk or otherwise be mobile within the office setting and in the field, and while representing the organization in the community. The employee is occasionally required to travel to other locations.
Ability to bend and lift 35 pounds.
Travel:
Due to PWA having various office locations, travel from home to workstations is not reimbursable. This position may require inner-city travel for work-related services.
