AMI Housing is Hiring a Case Manager Near Truckee, CA

Organization:

Founded in 2003, AMI Housing is a California-based nonprofit organization whose mission is to enhance the lives of people with mental illness through housing, employment, and supportive services. To house and support the most vulnerable residents of the Sierra region.

Position Summary:

North Tahoe-Truckee Homeless Services, a subsidiary of AMI Housing, Inc., is seeking 1 full-time Service Coordinators to work in our Truckee office. Service Coordinator's provide case management services solely to the unhoused community. Service Coordinators uses Housing First Principles, Harm Reduction and Motivational Interviewing models of case management that are client driven, solution focused and facilitate change and movement toward greater stability and well-being. Specific duties may include, but are not limited to home visits, intakes, tracking of services, and assisting in program development.

Essential Responsibilities:

  • Provide impartial and non-biased consultation, advocacy, and review of an individual's or family’s needs to support successfully becoming housed.
  • Provide case management and coordination to program participants.
  • Conduct intakes and provides assessments.
  • Maintains confidential case notes on each individual client.
  • Completes all documentation in a timely manner.
  • Develops and maintains cooperative working relationship with other agencies and service providers.
  • Serves as link between client and resources.
  • Provides reporting and tracking information for data management systems.
  • Determines client eligibility for homeless housing services.
  • Ensures compliance with federal, state, agency and/or funder guidelines.
  • Assists in program development work.
  • Attend weekly meetings as needed.
  • Supervision with Clinical Social Worker
  • Other duties as assigned.

Education and Background:

  • Bachelor’s degree or an associate degree with 2 years experience in a social services related field, OR a high school diploma and 4 years experience in a social service related field.
  • Bilingual fluency (English/Spanish) is preferred, pay differential with proof of oral and written proficiency.
  • Complete and pass pre-employment background checks (Fingerprints, drug and alcohol testing, and criminal background checks.
  • Posses valid driver's license and reliable transportation.

Work Experience may include:

  • 2 years experience in providing case management/service coordination with individuals experiencing symptoms of a mental illness, substance misuse, and/or homelessness
  • Experience working within some housing programs, such as HUD Section-Plus care, HUD Housing Choice, HUD Permanent Supportive Housing, and VASH Veterans Affairs Supportive Housing, is preferred.
  • Experience working with individuals with mental illness, drug/alcohol abuse, homelessness, or other social services.
  • Experience gathering and entering data into information systems, such as Coordinated Entry and Homeless Information Management System (HMIS).

Computer Proficiency:

  • Strong computer proficiency (i.e., Microsoft Word, Excel, PowerPoint, Outlook, SharePoint, Microsoft Teams, Zoom, Adobe, DocuSign, Google Docs, Internet, and Calendar) to complete daily duties within the expected timeframes with little to no supervision.
